Lymph node transplant

Lymph node transplant may be considered MEDICALLY NECESSARY when ALL of the following are met:

Diagnostic criteria: Signs and symptoms consistent with lymphedema as determined by a certified lymphedema therapist AND a diagnosis of stage II lymphedema (ISL) or equivalent; for unilateral disease volumetry differential (circumferential measurements and/or perometry) >10% if affected extremity is the dominant extremity or >7% if the affected extremity is non-dominant; for bilateral disease lymphoscintigraphy demonstrating minimum one-hour delayed transit to first-level (axillary for upper extremity or inguinal for lower extremity) nodes or a dermal backflow pattern.see thresholds

Eligibility prerequisites: BMI < 35 kg/m2; completed a course of conservative lymphedema therapy defined as compression therapy plus one or more of manual lymphatic drainage, complete decongestive therapy, pneumatic compression pump, or targeted exercises totaling a minimum of 20 hours/week for 6 months; demonstrated ability to tolerate post-surgical compression therapy and physical therapy as documented by the treating lymphedema provider; no listed exclusions (see exclusions).

Lymphovenous bypass

Lymphovenous bypass may be considered MEDICALLY NECESSARY when ALL of the following are met:

Diagnostic criteria: Signs and symptoms consistent with lymphedema as determined by a certified lymphedema therapist AND a diagnosis of stage II lymphedema (ISL); for unilateral disease volumetry differential (circumferential measurements and/or perometry) >10% if affected extremity is dominant or >7% if non-dominant, OR lymphoscintigraphy showing minimum one-hour delayed transit to first-level nodes or dermal backflow, OR bioimpedance (L-Dex) differential of at least 10 units (alternative quantitative measure).see thresholds

Debulking of a limb may be considered MEDICALLY NECESSARY when ALL of the following are met:

Diagnostic criteria: Signs and symptoms consistent with lymphedema as determined by a certified lymphedema therapist AND a diagnosis of stage II lymphedema (ISL); for unilateral disease volumetry differential (circumferential measurements and/or perometry) >10% if affected extremity is dominant or >7% if non-dominant, OR bioimpedance (L-Dex) differential of at least 10 units; for bilateral disease lymphoscintigraphy showing minimum one-hour delayed transit to first-level nodes or dermal backflow.see thresholds

Imaging requirement: MRI imaging findings consistent with moderate to severe fat hypertrophy.

Conservative therapy: Completed a course of conservative lymphedema therapy defined as compression therapy plus manual lymphatic drainage, complete decongestive therapy, pneumatic compression pump, or targeted exercises for a minimum of 20 hours/week for 6 months; demonstrated ability to tolerate post-surgical compression therapy and physical therapy.

Surgical eligibility narrative criteria

Surgical treatment is considered when ALL of the following general conditions are met (narrative evidence basis):

Conservative therapy failure: Patient has documented failure of appropriate conservative management, including education, skin hygiene, weight management, limb elevation, manual lymphatic drainage, compression garments, decongestive therapy, and pneumatic pumps.

Appropriate surgical options: Procedures that may be considered include debulking/liposuction for volume reduction, vascularized lymph node transfer (VLNT), and lymphovenous anastomosis (LVA); procedure selection should be based on clinical staging, goals, and documented donor and recipient sites.

Expected benefit documented: Documentation of anticipated benefits such as limb volume reduction, decreased infection incidence, improved quality of life, and a plan for postoperative compression and follow-up; evidence demonstrates volume reduction and reduced infections across procedures.

Exclusions and contraindications include active infection of the affected extremity (cellulitis/erysipelas); pregnancy; documented dye anaphylaxis; lipedema without lymphatic dysfunction; significant venous disease such as deep vein thrombosis or superior vena cava syndrome; medication-induced swelling; and uncontrolled comorbidities including congestive heart failure, liver disease (for example, cirrhosis or hypoproteinemia), and nephropathy including end-stage renal disease. Transient lymphedema (swelling occurring up to six months after the last oncologic treatment) is distinguished from persistent lymphedema and should be considered when evaluating eligibility.

Use of lymphovenous bypass specifically for the prevention of lymphedema is considered investigational and is excluded from coverage; this clarification was added to the policy in 6/2024.

If the explicit medical necessity criteria for a listed procedure (vascularized lymph node transplant, lymphovenous bypass, or debulking/liposuction) are not met, that procedure is considered investigational and therefore not medically necessary. For inpatient settings, precertification/preauthorization is required for all products when the procedure is performed inpatient. For outpatient procedures, prior authorization requirements vary by product: Commercial Managed Care (HMO and POS) — outpatient prior authorization is required; Commercial PPO and Indemnity — outpatient prior authorization is required; Medicare HMO Bluesm — outpatient prior authorization is not required; Medicare PPO Bluesm — outpatient prior authorization is not required. Lymphedema is a chronic disorder characterized by accumulation of subcutaneous fluid and fat in the affected tissues, producing limb swelling, discomfort, functional impairment, recurrent infections, and decreased quality of life. First-line management is conservative therapy (education, skin care, weight management, limb elevation, manual lymphatic drainage, compression garments, pneumatic pumps, and targeted exercises).
