The health plan allows reimbursement for a procedure or service billed for a single member on a single date of service by the same provider up to the maximum number of units allowed unless other provider, state, federal contracts, or requirements indicate otherwise.

The determination of appropriate multiple units uses claims processing logic, the CPT/HCPCS code nomenclature, CMS Medically Unlikely Edits (MUEs), industry standards, and clinical ability to perform/report the service more than once on a date or within a date span.

ALL of the following

- The procedure code description includes the word(s) bilateral or unilateral.
- The procedure code description specifies unilateral and there is a separate CPT code for the bilateral service or an add-on code for additional services (the unilateral code cannot be submitted more than once on a single date of service).
- The procedure code description or associated parenthetical statement specifies a time frame.
- The procedure code description implies multiplicity or the total number of clinically reasonable/permissible performances on a single date of service is limited.
- The procedure is typically not performed more than once on a single date of service even if submitted with multiple units.
When a procedure code is submitted with multiple units but only a single unit is acceptable, reimbursement will be based on only one unit; modifiers will not override this edit.

ALL of the following

Unit/frequency edits are applied pre-adjudication using both the unit field and multiple line-item submissions.
Frequency edits will be applied to a base code that has a related add-on code listed in CPT Appendix D; in such cases the base code is eligible for reimbursement only once per date of service.
Frequency edits may be applied across dates of service by accumulating claim history (previous, current, subsequent dates of service) to enforce the limit.
