Documentation requirements that must be met to support billed services for episodes of care; noncompliance may lead to denial or recoupment.

Documentation must be legible to someone other than the writer; include member identification on each page; be complete and dated; and each entry must include author identification (handwritten signature, unique electronic identifier, or initials and rendering provider credentials, as applicable).

Each record must support the services billed and the level of care provided on each unique date; working diagnoses and treatment plans must be consistent with findings and test results.

Minimum elements to be included when applicable: consent forms; health history including drug allergies; physical examinations; physician orders; immunization records; medications prescribed; emergency care; smoking, alcohol and substance use history; face-to-face evaluations; progress notes; referrals; consultation reports; laboratory and imaging reports (including X-ray); surgical reports; admission and discharge dates and instructions; preventive services; and evidence of coordination of care between primary and specialty physicians.

Timeliness expectations: timely entry at the time of service or shortly thereafter and should not exceed 30 days; signature date within 30 days of the date of service; time must be documented when applicable and signature time additionally documented for hospital services.

Billing and coding support: documentation should support procedure and modifier usage and standard correct coding; depending on the episode of care, more specific documentation in compliance with federal/state regulations and Joint Commission elements may be required.

Consequences if documentation is insufficient or not provided

The health plan may request additional documentation (including medical records or other documentation not directly related to the member) to support claims.
If requested documentation is not provided or does not support the services billed, the plan may deny the claim and recover/recoup previously paid amounts; the plan is not liable for interest or penalties when payment is denied or recouped due to provider failure to submit documentation.

Reporting note: when testing is performed over several days, all testing time should be reported on the last date of service.

Scope exclusion: this policy does not apply to facility providers.
