Coverage and edit application rules

Claims are adjudicated using CCEG measures; specific outcomes depend on edit results and plan benefits.

Code editing software, CMS NCCI edits (including PTP and MUE), OCE, clinical criteria, and claims processing platform edits are applied to evaluate claims for coding accuracy and adherence to standards and plan benefits.

Code editing software and CCEG measures are updated as needed to incorporate new codes, code definition changes, and edit rule changes; updates are applied to all claims submitted after the configuration implementation date.

Evaluation and Management (E/M) claims may be automatically adjudicated using a coding algorithm based on diagnosis codes and level of complexity/severity reported on the claim.

No retrospective payment changes, adjustments, or refund requests will be made when processing changes result from new code editing rules within a module update; members must not be balance billed for denials or reductions due to CCEG measures.

ALL of the following

Procedure-to-procedure (PTP) edits are enforced.
Medically Unlikely Edits (MUE) are enforced; when a service unit exceeds an MUE, the claim line(s) will be denied.

The health plan will not reimburse in the event of a conflict with CCEG.

Provider Responsibilities, Authorization, and Billing Guidance

Prior Authorization

Follow authorization, medical necessity, and correct coding/billing requirements

Providers must follow applicable authorization and medical necessity guidelines and ensure services meet authorization and medical necessity criteria appropriate to the procedure, diagnosis, and member's state of residence. Use proper billing and submission guidelines, including industry-standard CPT, HCPCS, and revenue codes, and ensure codes billed are fully supported in the medical record and/or office notes. Failure to follow authorization, medical necessity, or coding/billing guidelines may result in claim rejection, denial, recovery, and/or recoupment.

Obtain any required authorizations and document medical necessity per the member's benefit plan and state/federal mandates.
Bill with correct CPT, HCPCS, and revenue codes; ensure documentation in the medical record supports the codes billed.
Adhere to standard billing/submission practices to avoid claim rejection, denial, or recovery.

Editing (definition)

Definition of EditingThe practice or procedure by which one or more adjustments are made to CPT® or HCPCS codes on a claim resulting in partial payment, payment based on different codes, reduced payment via multiple procedure logic, denial of one or more codes, or any combination of these outcomes.
