Medical necessity criteria

Covered when ALL of the following are met

Primary clinical necessity: Placement of veneers must be necessary, appropriate and meet generally accepted standards of care and have a reasonable expectation for success.

Tooth structural requirements: To be considered for a labial veneer, the tooth must demonstrate significant loss of tooth structure and/or involvement of one or both incisal angles or cusp tip of a canine; fully intact lingual surfaces on anterior teeth that can support veneer placement are required when there are large areas of decay; when decay is too extensive a full crown may be the more appropriate restoration.

Degree of tooth breakdown must be documented by report, clinical chart notes, and/or scanned or photographic images upon request.

Radiographic requirement: A current (within 12 months) pretreatment periapical radiographic image showing the apex is required to evaluate for lack of pulpal and/or periapical pathosis.

Veneer placement is not appropriate on teeth with uncontrolled or untreated periodontal disease because poor periodontal health compromises long-term prognosis; documentation of definitive periodontal treatment history is required when assessing eligibility.

Patients with severe bruxism or grinding are considered poor candidates for veneers since the parafunctional forces place undue stress on the restoration and compromise longevity. In addition, restorative procedures intended to repair complications from wear, attrition, abrasion, erosion, or abfraction do not meet contractual criteria and are excluded from coverage.

Required documentation includes a current (<= 12 months) pretreatment periapical radiograph showing the apex for evaluation of pulpal and/or periapical pathosis, and documentation of periodontal health and long-term prognosis (history of definitive periodontal treatment when applicable). Upon request, provide clinical chart notes, scanned or photographic images demonstrating degree of tooth breakdown.

Veneer requests may be denied when procedures are performed for purely cosmetic reasons or to correct congenital/developmental defects or tooth size discrepancies. Denials are also common when periodontal disease is uncontrolled, when teeth demonstrate severe bruxism/grinding, when decay extent would be better managed with a full crown, or when restorative needs relate to wear/attrition/abrasion/erosion/abfraction rather than clinically necessary reconstruction.

Before considering veneers, providers should confirm that the tooth has fully intact lingual surfaces able to support a veneer when large areas of decay are present; when decay is too extensive a full crown may be the more appropriate restoration. A current (within 12 months) pretreatment periapical radiograph showing the apex is required to evaluate for pulpal or periapical pathology, and documentation of the degree of tooth breakdown (reports, chart notes, or images) must be available upon request.
