Medical necessity criteria for periodontal surgical procedures

Covered when ALL of the following are met

Clinical necessity and prior therapy: Periodontal health cannot be achieved or maintained nonsurgically; initial non-surgical therapy (e.g., scaling and root planing) has been completed with a minimum four-week healing period prior to any surgical treatment.

Documentation: Provide labeled and dated 6-point periodontal charting within 12 months (pre- and post-non-surgical therapy) and diagnostic-quality radiographs (within 12 months) demonstrating horizontal and/or vertical osseous defects as applicable.

Indications (examples): Procedures to eliminate periodontal pockets and recontour alveolar bone; to expose or lengthen the clinical crown; to remove enlarged gingival tissue to restore an anatomically correct gingival relationship; to allow proper contour of a restoration; to manage a fractured tooth extending subgingivally; or to increase access to the root surface when previous non-surgical attempts have been unsuccessful, particularly on a surface adjacent to an edentulous/terminal tooth area or when probing depths are greater than 5 mm.

Anatomic limits: Limited to natural teeth only; only teeth with favorable periodontal outcomes will be considered for benefit.

Frequency and quantity limits: Generally limited to one periodontal surgical procedure per quadrant and to two quadrants per date of service; generally limited to once every 36 months; benefits are group contract dependent. Exceptions may be allowed on a case-by-case basis with additional information requested.

The following circumstances are explicitly excluded from coverage for periodontal surgical procedures: procedures performed solely for cosmetic/aesthetic purposes; patients with an uncontrolled underlying medical condition; and procedures intended for the correction of altered passive eruption. Additionally, procedures are not indicated when patients have been non‑compliant with prior periodontal therapies, when treatment involves areas with proximal contact that can be managed non‑surgically, or when the clinical situation is inclusive with other surgical procedures in the same anatomical area on the same date of service. Benefits for primarily cosmetic procedures are not benefited and coverage is group contract dependent.

Revision or secondary periodontal surgical procedures submitted within the plan’s frequency limitations are subject to denial rules based on provider ownership: when submitted by the same dentist/dental office the claim is DISALLOWED; when submitted by a different dentist/dental office the claim is DENIED. Providers should verify applicable frequency limitations in the member’s contract before submitting a revision procedure to avoid these disallowance/denial outcomes.

Required non-surgical therapy must be completed prior to surgical therapy. Initial non-surgical periodontal therapy (for example, scaling and root planing) must be completed and allow a minimum four-week healing period before any periodontal surgical treatment to permit reassessment of periodontal status.

Therapy completion and timing

Therapy completion and timing — surgical procedures will be considered only after completion of initial non-surgical therapy and an adequate healing interval (minimum four weeks) to allow proper assessment of periodontal status. Periodontal surgical procedures are limited to natural teeth with pocket depths of 5 mm or greater and limited to two quadrants per date of service unless an exception is approved.

Clinical Background

Periodontal surgical procedures are indicated when periodontal health cannot be achieved or maintained nonsurgically. Typical clinical indications include eliminating periodontal pockets, recontouring alveolar bone, exposing or lengthening clinical crowns, removing enlarged gingival tissue to restore anatomy, facilitating proper restoration contour, addressing a fractured tooth that extends subgingivally, or increasing access adjacent to an edentulous/terminal tooth area when non‑surgical methods have been unsuccessful. These procedures are considered only after completion of initial non‑surgical therapy (scaling and root planing) with a minimum four‑week healing period and when documentation (labeled 6‑point periodontal charting and diagnostic radiographs) supports the presence of moderate to deep probing depths and osseous defects.
