General coverage conditions: Services must be provided by a Dentist exercising prudent clinical judgment, be in accordance with generally accepted standards of dental practice, be effective for the patient's dental injury or disease, not primarily for convenience or cosmetic, and not more costly than an alternative service

Clinical indicators for treatment: Diagnosis must be supported by clinical exam and radiographic evidence; clinical signs include swelling, redness, bleeding on probing, increased probing depth, suppuration, implant mobility, and percussion/occlusion findings> 5 mm

Probing depth > 5 mm, bleeding and/or suppuration on probing, and loss of supporting bone indicate peri-implantitis; radiographic exam used to differentiate mucositis from peri-implantitis

Coverage of services for peri-implant defect management is subject to the member's contract and applicable plan terms. Determinations that a service is medically or dentally necessary following clinical review do not by themselves guarantee that the service is a covered benefit payable by the dental plan. Contractual coverage limits, eligibility, and benefit-specific provisions may restrict payment even when clinical criteria are met.

Prior Authorization / Submission Requirements

Prior authorization is required for surgical management of peri-implant defects and for services where the plan’s contract or benefit design requires prior review. Requests must include complete submission documentation (see items) to permit clinical review; incomplete submissions may delay authorization.

Include most recent, dated, and properly identified pretreatment radiographic image(s) that show the radiographic apex.
Provide clinical chart notes and intra-oral photographs documenting signs and symptoms (swelling, redness, bleeding on probing, suppuration, implant mobility, percussion changes, occlusal findings).
Include current dated 6‑point periodontal charting including implant mobility and history of prior periodontal therapy.
Specify proposed procedure(s) with corresponding CDT codes and clinical rationale for surgery vs nonsurgical management.

Services Determined Medically/Dentally Necessary

The plan reviews requests to determine if services are medically or dentally necessary per contract terms and clinical standards. A determination that a procedure is medically or dentally necessary by clinical review does not guarantee it is a covered benefit — coverage is subject to the member’s contract and benefit limitations.

Clinical indications considered include documented signs of peri-implantitis or peri-implant mucositis (eg, probing >5 mm, bleeding and/or suppuration on probing, radiographic bone loss, implant mobility).
Coverage decisions reference member-specific contract language; non‑covered services or limitations in the member’s plan may apply even when clinically necessary.

Therapeutic Approach Guidance

Therapeutic approaches may be surgical or nonsurgical and should focus on infection control, detoxification of the implant surface, and regeneration of lost support when indicated. Selection of modality should be based on clinical findings, defect morphology, and prior response to therapy.

Nonsurgical options: systemic antibiotics alone; mechanical debridement with or without systemic antibiotics; mechanical debridement with localized drug delivery and adjunctive chlorhexidine rinses; mechanical debridement with LASER decontamination.
Surgical options: surgical debridement; surgical debridement with guided bone regeneration (GBR) including bone grafting and, when indicated, barrier membranes or biologic materials; apically positioned flap when clinically indicated; implant removal when non‑restorable.
Considerations: use radiographs and clinical probing to differentiate peri-implant mucositis from peri-implantitis and to guide whether regenerative procedures are appropriate.

Documentation requirements are essential for clinical review and payment determination. Failure to submit required documentation may result in denial or delay.

Required: dated pretreatment radiograph(s) including the apex, clinical chart notes, intra‑oral photographs, and current 6‑point periodontal charting with implant mobility.

Clinical Background

Peri-implant defects encompass inflammatory conditions of the soft and hard tissues around osseointegrated dental implants, commonly described as peri-implant mucositis (soft tissue inflammation only) and peri-implantitis (inflammation with loss of supporting bone). Diagnosis relies on clinical signs such as swelling, redness, bleeding or suppuration on probing, increased probing depths (for example, greater than 5 mm is a commonly noted threshold of concern), and implant mobility, together with radiographic evaluation to document bone loss and to distinguish mucositis from peri-implantitis. Management aims at infection control, implant surface decontamination, and—when indicated—regeneration of lost support using nonsurgical or surgical techniques selected according to accepted standards of dental practice.
