Coverage Criteria for Mucogingival Surgery and Soft Tissue Grafting

Medical necessity criteria for mucogingival surgery

Covered when ALL of the following are met:

Primary indications: Progressive gingival recession or loss of attached gingiva with root exposure; absence or reduced keratinized attached gingiva; periodontal pocket probing beyond the mucogingival junction; high frenum attachments; inadequate vestibular depth; chronic marginal inflammation or root sensitivity may influence need.

Documentation requirements: Prior to surgery document current dated periodontal chart (within 12 months) including millimeters of recession (CEJ to gingival margin), millimeters of attached gingiva, six-point pocket depths, history of progressive recession within prior 12 months, number of teeth affected, notation of high frenum attachments (C notation if present), photographic documentation, radiographs as indicated, relationship to cervical caries/restorations, and a statement that procedure is not cosmetic.

Anatomic threshold (unless extraordinary circumstances): Recession of at least 2 mm of keratinized gingival tissue with one millimeter or less of attached gingiva remaining.

Extraordinary circumstances allowing surgery despite thresholds: Frenum involvement, chronic inflammation, or other documented exceptional reasons may justify surgery even if anatomic threshold is not met.

Scope and limits: Procedures considered only for periodontal defects on natural teeth; benefits are group contract dependent and generally limited to one periodontal surgical procedure in a 36/60 month period per single tooth or multiple teeth in the same quadrant; frenectomy/frenuloplasty is inclusive when performed same date/area as a soft tissue graft; dermal matrix materials are denied when used for soft tissue grafting.

Mucogingival surgery and soft tissue grafting are considered inappropriate when the member demonstrates good oral health with effective plaque control and no evidence of plaque buildup. Clinical and experimental studies indicate that when plaque is controlled there is no minimum width of keratinized gingiva required to prevent periodontal disease, and therefore grafting is not indicated in these circumstances.

Additionally, dermal matrix materials are denied when used for soft tissue grafting and should not be submitted for coverage as part of mucogingival procedures.

Mucogingival surgery and grafting are not medically necessary when the clinical exam documents controlled plaque and overall good oral hygiene with no plaque buildup. In this setting there is no required minimum keratinized gingival width and surgical intervention is inappropriate.

When submitting claims or prior authorization requests, provide documentation demonstrating the presence of plaque-related disease or other qualifying clinical indications; absent such findings, requests should be expected to be denied as not medically necessary.

Mucogingival conditions that may warrant corrective surgery include progressive gingival recession with root exposure, loss or reduced amounts of keratinized attached gingiva, periodontal probing depths extending beyond the mucogingival junction, high frenum attachments, and inadequate vestibular depth. Chronic marginal inflammation and root sensitivity are additional clinical factors that may influence the need for treatment.

Surgical consideration is guided by documented anatomic thresholds (for example, recession of at least 2 mm with ≤ 1 mm of attached gingiva remaining) unless compelling circumstances exist (such as significant frenum involvement or persistent inflammation). Benefits are group contract dependent and generally limited to one (1) periodontal surgical procedure per tooth or quadrant in a 36/60 month period.
