Coverage and Medical Necessity Criteria

Medical necessity criteria for mucogingival surgery

Covered when ALL of the following are met

General criteria: Clinical indications include progressive gingival recession with loss of attached gingiva and concomitant root exposure, absence or reduced amounts of keratinized attached gingiva, probing depths extending beyond the mucogingival junction, high frenum attachments, or inadequate vestibular depth; benefits are group contract dependent.

Documentation criteria: A current (within 12 months) pretreatment periodontal chart must be documented prior to mucogingival surgery, including millimeters of recession (CEJ to gingival margin), millimeters of attached gingiva, 6-point pocket depths per tooth; history of progressive recession within 12 months; notation of high frenum attachments; number of teeth affected; photographic documentation as required; current (within 12 months) pretreatment radiographs when indicated; indication of root sensitivity; relationship to cervical caries/restorations; and a narrative rationale addressing keratinized attached gingiva, root coverage, sensitivity, and/or cosmetics.<= 12 months for documentation

Anatomic thresholds: Except for extraordinary circumstances (e.g., frenum involvement or chronic inflammation), mucogingival surgery for correction of recession defects is considered only when periodontal charting indicates a minimum of 2 millimeters of gingival recession and 1 millimeter or less of keratinized attached gingiva.recession >= 2 mm and keratinized attached gingiva <= 1 mm

Tooth eligibility: Mucogingival surgery will be considered for treatment of periodontal defects on natural teeth only.

Frequency limit: Benefits are group contract dependent but generally limited to one periodontal surgical procedure in a [36/60] month period per single tooth or multiple teeth in the same quadrant.

Inclusive procedures: Frenectomy or frenuloplasty performed in the same area on the same date as a soft tissue graft is considered inclusive to the graft procedure.

Biologic materials such as dermal matrix products are not benefited when submitted in conjunction with soft tissue grafting for mucogingival procedures. This applies regardless of whether the grafting procedure is autogenous or non‑autogenous; inclusion of biologic dermal matrix materials with a soft tissue graft claim does not create a separate benefit. Additionally, mucogingival surgery and grafting may not be necessary when there is good oral health with maintained plaque control, in which case procedures for root coverage or increasing keratinized gingiva are considered inappropriate.

Procedures intended solely for cosmetic purposes are considered not medically necessary and are not eligible for benefit. Likewise, procedures performed primarily as preparation for a non‑covered or denied service are inappropriate and may be denied. Surgery is also not indicated when clinical criteria (including required documentation or minimum anatomic thresholds) are not met; benefits are contingent on meeting generally accepted standards of dental practice.

Billing Rule

Do not submit biologic dermal matrix materials with soft tissue grafting for benefit

Biologic dermal matrix materials are not benefitted when submitted in conjunction with soft tissue grafting; do not bill or expect benefit for these materials with graft procedures.

Denial Risk

Risk of denial if standards, anatomic thresholds, or documentation are not met

Services may be denied if they do not meet generally accepted standards of dental practice, are primarily cosmetic, preparatory for a non-covered service, or lack the contractually required justification and documentation.

Surgery considered inappropriate (not medically necessary) when performed solely for cosmetics or when good oral health with no plaque buildup is present.
Lack of required documentation, failure to meet minimum anatomic thresholds (recession < 2 mm or attached gingiva > 1 mm except in extraordinary circumstances), or procedures performed to prepare for a non-covered service can trigger denial.
