Adjunctive use after definitive therapy: Local delivery of antimicrobial agents is used as adjunctive therapy to treat refractory pockets following completion of initial definitive non-surgical periodontal therapy (scaling and root planing) or in conjunction with periodontal maintenance.

Timing: LDCA may be performed six weeks to six months after scaling and root planing or periodontal surgery.

Site characteristics: Target sites are isolated/refractory periodontal pockets >= 5 mm with persistent signs of inflammation, spontaneous bleeding or bleeding on probing, suppuration and/or increasing clinical attachment loss.>= 5 mm

Local delivery of antimicrobial agents (LDCA) are considered an adjunctive therapy to definitive non‑surgical periodontal treatment and periodontal maintenance. Coverage is appropriate only when all relevant criteria are met, including that scaling and root planing has been completed and a sufficient healing interval has elapsed — LDCA may be performed six weeks to six months after scaling and root planing or periodontal surgery.

Target sites must be isolated/refractory periodontal pockets ≥ 5 mm with persistent signs of disease such as inflammation, spontaneous bleeding or bleeding on probing, suppuration, increasing pocket depth and/or increasing clinical attachment loss. A current, dated, post‑maintenance or post‑surgery, 6‑point periodontal charting within 12 months is required to support the clinical rationale for LDCA placement.

LDCA is not intended for use where multiple sites of recurrent or residual inflammation persist after scaling and root planing; in such cases more extensive periodontal modalities (for example, periodontal surgery) should be considered instead. Only teeth demonstrating favorable periodontal outcomes will be considered for benefit. Use in medically compromised patients will be reviewed on a case‑by‑case basis and requires physician documentation.

Billing Rule

Coding and submission

Providers must submit appropriate CDT procedure codes and supporting clinical documentation when billing for localized delivery of chemotherapeutic agents (LDCA). Include a current (within 12 months) dated 6-point periodontal charting and detailed periodontal chart notes that document probing depths, bleeding on probing, suppuration, attachment loss, and site-specific justification for LDCA.

Submit CDT code(s) on the claim (examples: D4381 for localized delivery of antimicrobial agents; related scaling/root planing codes: D4341, D4342; maintenance and adjunct codes: D4910, D4999, D6081).
Attach dated 6-point periodontal charting (within 12 months) showing pockets ≥5 mm at treated sites and evidence of inflammation (bleeding on probing, suppuration) when billing LDCA.
Provide clinical notes documenting prior definitive non-surgical therapy (scaling and root planing) and timing: LDCA performed between 6 weeks and 6 months after therapy when indicated.
For medically compromised patients, include physician documentation to support use of LDCA for case-by-case review.
If LDCA is performed in conjunction with services that are non-covered or denied, related services may also be denied — do not bill for preparatory services that are tied to non-covered procedures.

Therapy sequencing

LDCA should be considered only as an adjunct after definitive non-surgical therapy (scaling and root planing) has been completed and adequate healing time allowed. Verify and document the sequence and timing of prior therapies to support medical necessity.

LDCA is typically indicated 6 weeks to 6 months after scaling and root planing or periodontal surgery.
Document that non-surgical therapy was completed and that treated sites remain active/refractory (≥5 mm pockets with signs of inflammation) before billing LDCA.

Clinical documentation must include a dated 6-point periodontal charting within 12 months of service. Charting should be post-periodontal maintenance or post-periodontal surgery when relevant and must clearly identify the specific sites treated.

A current (within 12 months) dated, post-periodontal maintenance and/or post-periodontal surgery 6-point periodontal charting is required.
Charting must show site-level probing depths, bleeding on probing, suppuration, and any documented changes in attachment level supporting an active/refractory site.

Prior Authorization

Medically compromised patients

Use caution with medically compromised patients — coverage for LDCA will be evaluated individually and requires supporting physician documentation to justify use in the context of comorbid conditions.

Submit physician documentation that explains the medical condition and rationale for LDCA in medically compromised members.
Coverage for these cases is determined on a case-by-case basis; absence of physician documentation may result in denial.

Denial triggers

Services may be denied when probing depths are shallow or when LDCA is billed with or in preparation for non-covered procedures. Ensure treated sites meet the criteria for active/refractory disease and that LDCA is not used for bone regeneration (considered experimental).

LDCA is not appropriate for periodontal probing depths <5 mm — shallow sites are a denial trigger.
If LDCA or related services are performed in conjunction with non-covered/denied procedures, all related services may be denied.
Use of LDCA for alveolar bone regeneration is considered experimental and investigational and will not be covered.

Background

Local delivery antimicrobial agents are used as adjunctive therapy to manage refractory isolated periodontal pockets that persist after adequate non‑surgical therapy (scaling and root planing) or are identified during periodontal maintenance. Refractory or active sites are typically described as pockets 5 millimeters or greater exhibiting signs of inflammation, bleeding on probing or spontaneous bleeding, suppuration, increasing pocket depth, or progressive attachment loss. Because most diseased sites respond to scaling and root planing, LDCA is reserved for those limited sites that remain problematic after an appropriate healing interval.
