Documentation Required

Provide periodontal charting and radiographs

Submit current (within 12 months) documentation including a dated 6-point periodontal chart showing pocket depths (minimum 5 mm), a current periodontal pocket probing chart performed after completion of non-surgical periodontal therapy, and pretreatment radiographs that include the periapical area and an undistorted image of the alveolar crest.

Follow quadrant and frequency billing limits: benefits are generally limited to two quadrants per date of service and to one periodontal surgical procedure in a [36/60] month period per tooth or multiple teeth in the same quadrant. Group-specific limits may vary, so verify the member’s contract before billing; exceptions may be allowed on a case-by-case basis.

Do not bill laser or electrosurgery use as a separate, additional benefit. Laser/electrosurgery is considered an adjunct to gingivectomy/gingivoplasty and is not eligible for separate reimbursement; laser use must also be within the FDA 510(k) cleared indications for that specific device.

Background and Evidence

Gingivectomy/gingivoplasty are indicated for pocket elimination and gingival recontouring in supra-bony pockets with normal bony configuration, and for removal of loose or diseased gingival tissue when non-surgical therapy (e.g., scaling and root planing) has not adequately removed subgingival deposits. Lasers are described as an expanding adjunctive tool in dentistry and, while they may aid incision and healing, their use is considered an adjunct to treatment and is not eligible for a separate benefit. Coverage and benefit limits (including whether procedures are considered incidental or cosmetic) remain group contract dependent and the policy emphasizes verification of member contract terms for final coverage determinations. Generally accepted standards of dental practice are: standards based on credible scientific evidence published in peer-reviewed dental literature; specialty society recommendations/criteria; the views of recognized dentists practicing in the relevant clinical area; and any other relevant factors from credible sources.
