Veneer qualification: 1. Placement of veneers must be necessary, appropriate and meet generally accepted standards of care.

Expectation of success: 2. Must have a reasonable expectation for success.

Periodontal health: 5. Periodontal health and long-term prognosis must be considered; veneers on teeth with uncontrolled or untreated periodontal disease are not appropriate; documentation of definitive periodontal treatment history is required.

Labial veneer tooth breakdown: 6. To be considered for a labial veneer: a) tooth must demonstrate significant loss of tooth structure and/or involvement of one or both incisal angles or cusp tip of a canine; b) degree of tooth breakdown must be documented by report, clinical chart notes, and photographs upon request.

Replacement/ frequency: 7. Benefits for replacement of defective veneers after initial placement are subject to contractual frequency limitations.

Exclusions specific: 8. Placement of veneers are not considered for: a) cosmetic purposes; b) correction of congenital/developmental problems; c) correction of tooth size discrepancies.

Wear-related restorations: 9. Restorative procedures for repair of complications from wear, attrition, abrasion, erosion, and abfraction do not meet contractual criteria for treatment.

Veneer placement will not be covered when performed solely for cosmetic purposes or to address esthetic-only concerns. The policy explicitly excludes veneers used to correct congenital or developmental problems and to correct tooth size discrepancies. Coverage depends on the member's contract language and any cosmetic dentistry exclusion; inclusion of procedure codes does not guarantee benefits.

Veneers are not appropriate for patients who demonstrate severe bruxism or grinding. The policy notes that these conditions place undue stress on the restoration and compromise the long-term prognosis, and therefore such patients should not receive veneers.

Restorative procedures intended to repair complications resulting from wear, attrition, abrasion, erosion, and abfraction do not meet contractual criteria for treatment under this policy and are considered not medically necessary. Similarly, replacement benefits for defective veneers are subject to contractual frequency limitations and are not intended to cover restorations driven by these wear-related processes.

Verify member benefits and any cosmetic dentistry exclusions in the contract; approval requires documentation that all listed clinical criteria are met and that the service is not excluded by the member's contract.

Providers must submit a complete pretreatment record package to support medical necessity for veneer placement. Include clinical findings, radiographs, periodontal status, and photographic documentation when requested.

Submit a current (<= 12 months) periapical radiograph showing the apex for each tooth considered for a veneer.
Provide chart notes and a report detailing the degree of tooth structure loss; include intraoral photographs when requested.
Document periodontal evaluation and history of definitive periodontal treatment; note any uncontrolled periodontal disease.
If replacement veneers are requested, include documentation of initial placement date and applicable frequency limits per contract.

Required Documentation

Required documentation must demonstrate the degree of tooth breakdown, absence of pulpal/periapical pathology, and suitability of the lingual surface to support a veneer. Photographs and reports should be recent and complete.

Periapical radiograph(s) dated within 12 months that clearly show root apices and absence of pulpal/periapical disease.
Clinical chart notes describing extent and location of tooth structure loss and rationale that veneer (not crown) is appropriate.
Intraoral photographs documenting incisal/occlusal and facial aspects and intact lingual surfaces when support is required.
Periodontal charting and documentation of any prior definitive periodontal therapy; note if periodontal disease is uncontrolled.
For replacement veneers, include prior restoration details and dates to assess contractual frequency limitations.

Denial Risk

Veneer requests that are solely cosmetic, for congenital/developmental corrections, or to correct tooth size discrepancies are not covered and risk denial. Requests without adequate supporting documentation (radiographs, photos, periodontal history) are at high risk for denial.

Do not submit veneers for purely esthetic or cosmetic indications — these are likely to be denied.
Veneers for congenital/developmental problems or tooth size discrepancies are not covered.
Veneers are inappropriate for patients with severe bruxism/grinding — such cases risk denial due to poor prognosis.
Incomplete or outdated radiographs, missing photographic documentation, or absent periodontal history increase denial risk.

Background

Veneers are described as a conservative restoration for anterior teeth intended when there is significant loss of tooth structure, including involvement of one or both incisal angles or the canine cusp tip. They are appropriate when teeth retain fully intact lingual surfaces able to support the veneer and when there is a reasonable expectation of success. Veneers are not suitable for minor incisal defects that can be managed by less invasive means, for teeth with uncontrolled periodontal disease, or for patients with severe bruxism because these conditions compromise long-term prognosis.
