Services that are performed primarily for the convenience of the patient or dentist, that are cosmetic in nature, or that are more costly than an effective alternative may not meet the plan's requirements for medical/dental necessity and therefore may not be covered. The plan evaluates whether a requested service meets generally accepted standards of dental practice and represents the lowest-cost option that appropriately addresses the patient’s condition when determining benefit payment.

For the purposes of this policy, generally accepted standards of dental practice include standards based on credible, peer‑reviewed dental literature, specialty society recommendations or criteria, the views of recognized practicing dentists in the relevant clinical area, and other relevant credible sources.

Procedures that are not performed in accordance with generally accepted standards of dental practice, or that are not effective in type, frequency, or extent for the patient’s condition, are considered not medically/dentally necessary and may be denied. The determination of necessity also takes into account contractual requirements and whether the requested service appropriately addresses the patient’s dental injury or disease.

The plan’s review will consider clinical evidence, specialty guidance, and recognized practitioner standards when assessing whether the type, frequency, or extent of a procedure is appropriate for the member’s condition.

Background

A dental prophylaxis is a routine preventive procedure performed to remove supragingival plaque, stain, and calculus from tooth surfaces using ultrasonic or hand instruments and includes observation of hard and soft tissues. Prophylaxis is intended to maintain gingival health and reduce factors that contribute to plaque build-up.

Individuals at higher risk for dental disease may require prophylaxis more frequently than every six months. When heavy plaque or calculus prevents a comprehensive oral examination, a full mouth debridement (D4355) may be required as a preliminary treatment to enable diagnosis and subsequent definitive care.
