Medical necessity criteria

Covered when ALL of the following are met (where specified) or under the listed circumstances:

General necessity: Core buildup is necessary when there is insufficient coronal tooth structure for retention of a separate extracoronal restoration and when it rebuilds internal tooth anatomy as prepared for a crown.

Posterior teeth covered only when: For posterior teeth: any of the following - a significant portion of missing tooth structure due to caries, fracture, or defective restoration; insufficient tooth structure coronal to the intended restorative margin for adequate retention; AND a crown or retainer is existing (to be recemented/rebonded) or is treatment planned.

Anterior teeth covered only when: For anterior teeth: any of the following - a significant portion of missing tooth structure due to caries, fracture, or defective restoration; insufficient tooth structure coronal to the intended restorative margin for adequate retention; AND a crown or abutment is existing (to be recemented) or is treatment planned.

Primary tooth in adult dentition: For a primary tooth within an adult dentition to be considered for a core buildup and/or post-and-core, radiographic images must demonstrate intact root structure, adequate periodontal support with no evidence of active periodontal disease, occlusal function with an opposing tooth, and no permanent tooth successor or unlikely eruption.

Periodontal and endodontic considerations: teeth with uncontrolled or untreated periodontal disease (loss of supporting bone), untreated or unresolved periapical or periradicular pathology, unresolved lesions near the pulp without planned endodontic therapy, overfill/underfill root canal obturation or poor condensation, or internal/external resorption may not be considered for benefit.

Core buildup and post-and-core procedures are not covered when submitted solely for pulp capping, pulp protection/insulation, undercut block-outs, or to enhance box-form/fillers to reduce the final restorative material. They are also not considered a covered benefit for repair of complications from attrition, abrasion, erosion, or abfraction, nor when submitted to support partial-coverage indirect restorations (for example, onlays, inlays, veneers, or partial crowns). Additionally, the diagnosis of Cracked Tooth Syndrome alone does not qualify a tooth for these procedures.

Core buildup and/or post-and-core procedures are not medically necessary for purposes that do not address the clinical indications for rebuilding coronal tooth structure, including pulp capping/insulation, undercut block-outs, or mere enhancement of box form/fillers. These procedures are also not medically necessary for correction of developmental or congenital defects, for repair of tissue loss from attrition/abrasion/erosion/abfraction, or when caries extends into the furcation. When radiographs or clinical findings do not demonstrate an indication, adjunctive documentation (clinical notes, intra-oral photographs, or an image after removal of an existing restoration) should be provided, but such documentation does not make medically unnecessary uses eligible for benefit.

Examples that may trigger denial include untreated or unresolved periapical/periradicular pathology, unresolved lesions near the pulp without planned endodontic therapy, internal or external resorption, overfill/underfill root canal obturation, uncontrolled or untreated periodontal disease with supporting bone loss, caries into the furcation, and crown-to-root ratios poorer than 1:1.

Prior Authorization

Depending on group contracts, a core buildup may be alternated to a one‑surface restoration in instances where root canal therapy is performed through a retained existing crown. Verify contract‑specific coverage rules and prior authorization requirements with the member's plan.

Clinical background

A core buildup is performed to restore coronal tooth structure when there is insufficient tooth structure to retain an extracoronal restoration and to rebuild the internal anatomy in preparation for a crown. Core build-ups are integral to both prefabricated and indirectly fabricated post-and-core systems. For posterior and anterior teeth, coverage is limited to situations with a significant portion of missing tooth structure (for example from caries, fracture, or defective restorations), insufficient coronal tooth structure coronal to the intended restorative margin for retention, and when a crown, retainer, or abutment is existing or treatment planned. Clinical and periodontal status, endodontic status, and crown-to-root ratio (ratios poorer than 1:1 may be denied) must be considered when determining appropriateness.
