Medical-Necessity Criteria

Appropriate Indications

Implant/abutment supported crowns and fixed partial dentures to replace missing teeth may be considered appropriate when ANY of the following are met:

Accidental traumatic injuries to sound, natural teeth resulting in extraction/avulsion
A pathologic disorder resulting in extraction
Congenitally missing teeth
Congenital disorders of teeth resulting in extraction

General Coverage Restrictions / Exclusions

Coverage is NOT appropriate when ALL of the following apply (contract dependent as noted):

Teeth are not missing (implant prosthetics to replace teeth that are present are not appropriate), including replacement of third molars
Replacement of multiple teeth by implant supported crowns or fixed partial dentures in the same arch when other less costly professionally acceptable dental services can adequately restore occlusion to function (not medically necessary unless specified by group contract)
Plans that contain a missing tooth clause: no benefit for replacement of missing teeth when implant/abutment supported crowns and fixed partial dentures are treatment planned; implant placement/restoration not covered for third molar sites

Pre-procedure and Documentation Requirements

Covered only when ALL of the following documentation and preparatory conditions are met:

A thorough dental evaluation, medical and dental history, full mouth imaging, and treatment plan completed prior to implant placement and restoration
Current (within 12 months), dated, diagnostic, preoperative radiographic images are providedwithin 12 months
