Medical Necessity and Coverage Criteria

General medical necessity

Covered when ALL of the following are met:

ALL of the following

Services consistent with the member's diagnosis or condition (Medically/Dentally Necessary).
Rendered in response to a life-threatening condition or pain; or to treat an injury, illness or infection related to the dentition; or to achieve a level of function consistent with prevailing community standards.
Dentist administering anesthesia must be appropriately educated, trained and licensed; additional individuals trained in Advanced or Basic Life Support (state-specific) must be present when advanced anesthetics used.
Medical documentation from a physician must support medical conditions that necessitate general anesthesia or conscious sedation when used for such conditions.

Local anesthesia

Policy position:

Administration of local anesthetic (traditional, electronic, buffered, etc.) is considered inclusive of all dental procedures and is not eligible for separate benefit unless a specific plan allows coverage.

Regional and trigeminal division block anesthesia

Policy position:

Regional and trigeminal block anesthesia may not be a covered service.

Deep sedation / General anesthesia / IV moderate (conscious) sedation / Non-intravenous conscious sedation

These procedures may be benefitted when performed in conjunction with covered dental services and when medical necessity criteria are met. To qualify, the member must satisfy ONE of the criteria below (ANY):

Qualifying clinical indications

The member is a child, up to the age defined by contract, with a dental condition requiring repairs of significant complexity (e.g., multiple restorations, pulpal therapy, extractions or combinations).
Age of child is defined by the member's contract; AAP/AAPD definitions referenced but plan follows contract/state age limits.
Member has physical, intellectual, or medically compromising conditions (e.g., intellectual disabilities, cerebral palsy, epilepsy, severe cardiac problems, hyperactivity) for which dental treatment under local anesthesia cannot be expected to provide a successful result.
Requires verification by appropriate medical documentation from physician/oral surgeon/dentist.
Member is extremely uncooperative, fearful, unmanageable, anxious, or uncommunicative with complex dental needs that should not be postponed and lack of treatment would result in oral pain, infection, tooth loss, or increased oral morbidity.

Inhalation nitrous oxide (D9230) / Analgesia

Nitrous oxide may be benefitted when appropriate in conjunction with covered dental services and when INDICATIONS are met; contraindications listed below:

Indications

Ineffective local anesthesia administration.
Treatment of dental anxiety.
Special needs patients.
Individuals who are uncooperative or behaviorally challenged.

Monitoring and documentation requirements

When anesthesia/sedation is provided, ALL of the following must be performed and documented:

A dentist or appropriately trained individual must remain in the operatory during active dental treatment to continuously monitor patient's vital signs until criteria for discharge to recovery area are met.

Monitoring must include observation of proper oxygenation (including color of mucosa) and continuous evaluation of skin or blood; pulse oximetry is appropriate and necessary.

Ventilation: chest movement must be continually observed; respirations must be verified and documented by appropriately trained personnel.

Circulation: blood pressure and heart rate must be evaluated pre-operatively, intra-operatively and postoperatively and documented.

Documentation: a complete anesthesia record must be maintained including names of all drugs administered, time of administration, dosages (local or other anesthetic drugs), and all physiological parameters monitored.

Documentation Required

Medical documentation to support anesthesia necessity

Submit medical documentation from physician/oral surgeon/dentist to support medical conditions necessitating general anesthesia or conscious sedation. Necessary documentation includes all associated radiographic images, progress notes, operative report and a complete anesthesia record indicating start and stop times and incremental drug administration and dosages.

If local anesthesia is ineffective, document failed attempts and the reason (for example: acute infection, anatomic variation, or allergy) and submit the documentation for review to support consideration of more advanced anesthesia.

Documentation Required

Continuous monitoring and recording

Continuously monitor and document physiologic parameters when anesthesia/sedation is provided. Monitoring must include observation of proper oxygenation (pulse oximetry and mucosal color), continuous evaluation of ventilation (chest movement and verified respirations), and circulation (blood pressure and heart rate) pre‑operative, intra‑operative and post‑operative. Maintain a complete anesthesia record listing all drugs administered with times and dosages.

Billing Rule

Local anesthetic administration is considered part of dental procedures and is not eligible for a separate benefit unless a specific plan allows separate coverage. Providers should bill codes D9210 or D9215 only when supported by the patient’s contract or the plan’s explicit coverage allowance.

Background and Definitions

This policy describes the continuum of sedation depth accepted by professional societies and used to define levels of anesthesia; the level is determined by the anesthetic agent's effect on the central nervous system per ASA/AAP/AAPD/ADA guidance.

Anesthesia practice must align with provider education, training, licensing and presence of additional personnel trained in Basic or Advanced Life Support when more advanced anesthetics (IV or inhalation) are used; state-specific requirements for sedation licenses, equipment inspection and CPR certification apply where mandated.

Monitoring, documentation and continuous attendance requirements must be met (continuous vital sign monitoring including pulse oximetry, ventilation and circulation checks, and a complete anesthesia record) and coverage is subject to member contract and any state-mandated age or certification rules for pediatric patients.

General Anesthesia: Drug-induced loss of consciousness where patients are unarousable and may require airway maintenance and positive pressure ventilation; cardiovascular function may be impaired.

Deep Sedation/Analgesia: Drug-induced depression of consciousness in which a person is not easily aroused but responds purposefully after repeated or painful stimulation; airway assistance may be required; cardiovascular function generally preserved.

Intravenous Moderate (conscious) Sedation/Analgesia: Drug-induced depression of consciousness where no airway interventions are necessary, spontaneous breathing is adequate, and the patient remains able to follow directions.

Minimal Sedation (Anxiolysis): Drug-induced state allowing normal response to verbal commands with possible impairment of cognitive function and coordination.
