Service is consistent with the member's diagnosis or condition
Service is in response to a life-threatening condition or pain; or to achieve a level of function to the dentition consistent with prevailing community standards for the diagnosis or condition; or to treat an injury, illness or infection related to the dentition
Service is in accordance with generally accepted standards of dental practice (specialty society recommendations, peer-reviewed dental literature, views of recognized dentists, other credible sources)

When general anesthesia or conscious sedation may be covered

Member must satisfy ONE of the following criteria:

ONE of

Member is a child (up to age defined by contract) with complex dental condition requiring significant repairs (e.g., multiple restorations, pulpal therapy, extractions or combinations of procedures)
Member has physical, intellectual, or medically compromising conditions for which dental treatment under local anesthesia cannot be expected to provide a successful result (includes but not limited to intellectual disabilities, cerebral palsy, epilepsy, severe cardiac problems, hyperactivity) and supported by appropriate medical documentation
Member is extremely uncooperative, fearful, unmanageable, anxious, or uncommunicative with complex dental needs that should not be postponed and for whom lack of treatment can be expected to result in oral pain, infection, loss of teeth, or other increased oral morbidity
Local anesthesia is ineffective for the member (e.g., due to acute infection, anatomic variations, allergy); failed attempts at local anesthesia must be documented and submitted for review

Monitoring and Documentation requirements

Anesthesia and sedation are covered only when monitoring and documentation requirements are met:

A dentist or appropriately trained individual must remain in the operatory during active dental treatment to monitor the patient's vital signs continuously until discharge criteria to recovery area are met
Monitoring must include observation of proper oxygenation (color of mucosa/skin) and oxygen saturation by pulse oximetry
Ventilation: chest movement must be continually observed and respirations verified and documented by appropriately trained personnel
Circulation: blood pressure and heart rate must be evaluated pre-operatively, intra-operatively and postoperatively and properly documented

Duration and office anesthesia

Additional documentation required when:

Office anesthesia in excess of 60 minutes for any dental or surgical procedure requires written rationale/documentation explaining the necessity
Necessary documentation includes associated X-rays, progress notes, operative report and a complete anesthesia record indicating start and stop times of incremental drug administration

Nitrous oxide indications and contraindications

Nitrous oxide inhalation may be covered when used in conjunction with covered dental services if ANY indication applies and NONE of the contraindications are present:

Monitoring and personnel requirements

Ensure appropriately trained personnel remain in the operatory during active treatment to continuously monitor the patient until discharge criteria are met. Document monitoring of oxygenation, ventilation and circulation. For IV or profound inhalation anesthesia, ensure additional individuals trained in Basic Life Support are present and, if required by State, have current equipment inspection certificates, sedation licenses and CPR certification.
