Medically/Dentally Necessary (General Definition)

Medically/Dentally Necessary (General Definition): Covered when the services meet the plan definition of medical/dental necessity and are consistent with the member's diagnosis or condition.

Medically/Dentally Necessary or Medical/Dental Necessity means Medical/Dental Services that are consistent with the Member's diagnosis or condition and either: in response to a life‑threatening condition or pain; to achieve a level of function to the dentition consistent with prevailing community standards for the diagnosis or condition; or to treat an injury, illness or infection related to the dentition.

Dental services are considered medically/dentally necessary when, in the judgment of a Dentist exercising prudent clinical judgment, they: are in accordance with generally accepted standards of dental practice; are effective for the patient's dental injury or disease in terms of type, frequency and extent; are not primarily for convenience; are not cosmetic; and are not more costly than an alternative service. Sources for generally accepted standards include specialty society recommendations, peer‑reviewed dental literature, and recognized practicing dentists in the clinical area.

Monitoring Requirements

Documentation and Operational Requirements for Advanced Anesthesia: All of the following must be satisfied.

ALL of the following

Medical conditions necessitating general anesthesia or conscious sedation must be supported by documentation submitted by the oral surgeon, dentist and/or physician administering the anesthesia.
If more advanced anesthetics (e.g., IV or inhalation) are used to induce more profound levels of anesthesia, additional individuals trained in Basic Life Support (BLS) for Healthcare Providers must be present in addition to the dentist/oral surgeon.
If required by state regulation, a current equipment inspection certificate, sedation license, and CPR certification must be maintained and available.
The dentist administering the anesthesia must remain in constant attendance with the patient until completion of the anesthesia procedure.
