Coverage Summary
Scope: This policy defines medical/dental necessity, coverage considerations, monitoring, documentation, and provider qualifications for anesthesia used in the dental setting, including local anesthesia, regional/trigeminal block, conscious sedation (intravenous and non-intravenous), deep sedation/general anesthesia, and inhalation nitrous oxide. Coverage of anesthesia is available only when the anesthesia is provided in conjunction with underlying dental or medical services that are covered under the member's benefit plan or when a state mandate requires coverage; plan/contract benefits should be verified at the time of service.
