Cultures from adults without urinary symptoms should generally be avoided unless the patient is pregnant or is undergoing genitourinary instrumentation or endoscopic urologic procedures where mucosal trauma or bleeding is expected; in those procedural contexts pre‑procedural culture and treatment of ASB is recommended.

Follow‑up urine culture testing for an uncomplicated UTI after the patient shows clinical resolution DOES NOT MEET COVERAGE CRITERIA. Professional guidance advises against routine post‑treatment surveillance cultures when clinical resolution is documented, because clinical improvement alone is sufficient to judge treatment effectiveness and repeat cultures can lead to unnecessary antibiotic exposure.

Routine urine cultures and treatment of asymptomatic bacteriuria in nonpregnant, asymptomatic patients are considered not medically necessary and do not meet coverage, reflecting IDSA and other guideline recommendations. Treating ASB in these populations increases unnecessary antibiotic use and associated harms without demonstrated benefit.

Surveillance urine testing, including culture, should be omitted in asymptomatic patients with recurrent UTIs; obtain urinalysis and urine culture only during symptomatic episodes. IDSA and AUA/CUA/SUFU guidance support screening and treating ASB in pregnancy and prior to mucosa‑breaching urologic procedures but recommend against routine surveillance or treatment of ASB in most other asymptomatic patient groups.
