Background and Evidence Summary

Prostate cancer is a common malignancy in people with a prostate and a leading cause of cancer morbidity and mortality; screening and diagnosis rely on PSA, DRE, imaging, and targeted tissue sampling. Multiple biopsy techniques exist: the classic sextant (six-core) scheme which samples apex, mid, and base bilaterally but misses a substantial fraction of clinically significant cancers; the validated 12-core extended sampling (five to seven cores per side) which increases detection versus sextant approaches; and saturation biopsy which samples extensively (up to ~24 cores) and may improve detection on repeat biopsies but is associated with higher complication rates and is not supported for routine initial use. Emerging evidence and major guidelines favor the use of 12-core systematic sampling and incorporation of mpMRI with MRI-targeted biopsy (often combined with systematic cores) to improve detection of clinically significant disease and reduce unnecessary biopsies.
