Presumptive Drug Screening (Urine) - Covered Indications

Presumptive urine drug screening MEETS COVERAGE CRITERIA when ANY one of the following indications is present:

Definitive Drug Testing - Covered Criteria

Confirmatory/definitive drug testing (up to seven drug classes) MEETS COVERAGE CRITERIA when ALL of the following are met:

Definitive testing general criteria: 1) Laboratory-based definitive testing is specifically requested and the rationale is documented by the patient's treating physician. AND 2) One or more of the following conditions are present: a) The presumptive screen result is inconsistent with the patient's history, clinical presentation, or statement (e.g., negative for prescribed medication, positive for an unprescribed prescription drug, or positive for an illegal drug); b) For diagnosis or monitoring of substance use disorder when accurate and reliable results are necessary for treatment decisions with frequency limits tied to abstinence duration (0–30 days: random definitive testing ≤1/week; 31–90 days: random definitive testing 1–3/month, no more than 3 definitive tests in one month; >90 days: definitive testing 1–3 tests every 3 months, no more than 3 definitive tests in a three‑month period); c) Monitoring individuals on opioid therapy to ensure adherence, guide treatment planning, or detect non‑prescribed opioids; d) When no appropriate presumptive test exists or the presumptive test does not adequately detect the specific drug/metabolite (e.g., specific agents within amphetamine, barbiturate, benzodiazepine, tricyclic antidepressant, or opiate/opioid classes, or synthetic/analog/designer drugs); e) To definitively identify specific drugs within a large family of drugs; f) When a definitive concentration is needed to guide clinical management.

This policy addresses clinical toxicology in the outpatient setting and does not apply to forensic testing, employment-related testing (including commercial driver licensing), urgent/emergency care testing, or testing that is part of per diem/case rate reimbursement. Providers should follow applicable state and federal laws when those circumstances apply and refer to benefit-specific rules when determining coverage.

Covered specimen types are limited. Only urine or oral fluid specimens will be covered except that blood specimens may be covered for patients with anuric chronic renal failure. Same-day duplicate testing from two different specimen types and blanket or routine standing orders for all patients are not reimbursed.

Routine screening for acetaminophen or salicylates in the psychiatric pre‑admission setting was judged to be of low clinical value in a multicenter retrospective study and described as 'unnecessary and wasteful.' Such indiscriminate screening without a clinical indication is not supported by the evidence cited in this policy.

Presumptive immunoassays have important limitations. Studies cited in the policy report substantial false negatives and variable cross‑reactivity; for example, enzyme immunoassays demonstrated an overall sensitivity of 78.5% versus LC‑MS/MS with opiate EIAs showing a reported 21% false‑negative rate. Because of these limitations, presumptive results should not be used alone to make definitive clinical decisions without confirmatory testing and expert interpretation.

The American Society of Addiction Medicine notes that quantitative drug concentrations in urine should not be used to determine adherence to a specific dosage or formulation regimen. Quantification may have selective clinical uses (for example, distinguishing ongoing excretion from new use), but overall urine concentration correlates poorly with administered dose due to pharmacokinetic and pharmacogenetic variability.

ASAM further recommends using definitive techniques (e.g., GC‑MS, LC‑MS/MS) when specific identification or quantification is needed, and emphasizes that quantification alone is insufficient to infer adherence or exact dose without clinical context and appropriate interpretation.

Obstetric guidance notes that routine universal urine drug screening in pregnancy is controversial. ACOG recommends using validated verbal screening tools as part of routine obstetric care and states that urine drug testing should only be performed in compliance with state laws and with patient consent, reserving UDT for situations where testing is clinically indicated and follow‑up confirmation is available.

When this policy conflicts with an applicable government policy (for example, a Medicare LCD/NCD or state Medicaid coverage rule), the government policy takes precedence and will be used to make coverage determinations for the affected member.

A new general exclusion was added to clarify that in all other situations not specifically addressed by the coverage criteria, presumptive drug screening and definitive drug testing do not meet coverage criteria (CC6). This exclusion means tests falling outside the policy's listed indications and conditions are not eligible for coverage.

Regarding pregnancy, ACOG does not endorse routine universal UDT and recommends validated verbal screening tools instead; universal urine screening in pregnancy is controversial and routine testing is not recommended over appropriate screening and informed consent.
