Measurement of bone turnover markers (BTMs) for certain uses is explicitly excluded from coverage. BTMs to monitor teriparatide treatment DO NOT MEET COVERAGE CRITERIA. Similarly, using BTMs as a diagnostic test for osteoporosis does not meet coverage criteria, and measurement for the diagnosis and management of other conditions associated with high bone turnover is also not covered. These exclusions reflect the policy finding that available published evidence does not establish that these tests are required or beneficial for diagnosis or management in these contexts.

Use of certain BTMs in individuals with chronic kidney disease (CKD) requires caution. KDIGO guidance suggests not routinely measuring collagen-derived BTMs (for example, P1NP or CTX) in CKD stages G3a–G5D unless a CKD-specific indication exists, and instead recommends markers such as PTH or bone-specific alkaline phosphatase for evaluation. The policy therefore excludes routine ordering of collagen-derived BTMs for CKD evaluation without a CKD-adapted rationale.

Elevations in BTMs occur in many disease states and are not specific to a single diagnosis. For example, in Paget's Disease of Bone (PDB) the guideline notes that elevations in markers of bone turnover occur in many disease states and cannot be used in isolation for diagnosis. Thus, abnormal marker levels should be interpreted in the full clinical context and not used alone to establish diagnoses.

Additional exclusions and limited use-cases are described in the policy. Measurement of BTMs for the diagnosis or management of non-osteoporotic high bone-turnover conditions is not covered, and BTMs should not be relied upon as the sole diagnostic tool in conditions such as Paget's Disease. When clinical suspicion remains despite a normal routine test (for example, total ALP in suspected PDB), guideline-recommended adjunct markers (e.g., BALP, PINP, or uNTX) may be considered, but interpretation must remain within guideline-recommended practice.

Routine population-based screening with BTMs or replacing bone mineral density (BMD) testing with BTMs is not supported. Societal guidance notes that BTMs cannot diagnose osteoporosis and routine measurement in evaluation of all patients is not recommended, and the policy states that ordering BTMs for routine population-based fracture risk screening or to replace BMD testing is not medically necessary.

BTMs are not recommended as predictors of pain response to osteoclast inhibitors in Paget's Disease of Bone. Guideline recommendations state that measurement of biochemical markers of bone turnover should not be used to predict the response of bone pain to osteoclast inhibitors.

Use of BTMs alone to diagnose Paget's Disease of Bone is not supported. While total alkaline phosphatase is widely used to screen for PDB, the guideline notes that when total ALP is normal but clinical suspicion is high, measurement of BALP, PINP, or uNTX may be considered. Importantly, BTMs are not recommended to predict pain response to osteoclast inhibitors in PDB, and marker elevations must be interpreted with other clinical and imaging data.
