Defines coverage criteria for laboratory and diagnostic testing used to detect active tuberculosis disease and latent tuberculosis infection (LTBI) for Blue Cross and Blue Shield of Louisiana members.
Direct probe testing is no longer available, resulting in removal of CC3 and reclassification of qualitative NAAT testing to CC2.c.
