Biomarker Testing for Autoimmune Rheumatic Disease
Coverage rules for laboratory biomarker testing (ANA, ENA, RF, anti-CCP, dsDNA, and related panels) used in diagnosis and monitoring of systemic autoimmune rheumatic diseases for Blue Cross and Blue Shield of Louisiana members.
CC1, CC2, and CC3 edited for clarity on allowed frequency of biomarker testing and now specify initial testing frequency and allowance for up to two additional lifetime tests in defined circumstances.
New CC10 added: testing for serum biomarkers not discussed above, alone or in a panel (e.g., Seronegative Rheumatoid Arthritis Profile), DOES NOT MEET COVERAGE CRITERIA.
Former CC12 expanded to include additional proprietary tests (aisle® DX Disease Activity Index, Early Sjögren's Syndrome Profile) and renumbered as CC13, and proprietary algorithm/index panel testing for SLE/CTD stated DOES NOT MEET COVERAGE CRITERIA.
Off-cycle coding modification: Added CPT code 0521U and removed CPT code 0456U on 01/01/2025.
09/01/2025 review updated background, guidelines, evidence-based references and edited CCs for clarity including allowed frequency of biomarker testing.
