Removed 'are undergoing evaluation for infertility' from CC1.d and revised CC1.d to require TSH testing for individuals capable of becoming pregnant who have experienced two or more pregnancy losses.
Added CC1.e.: 'TSH testing for individuals with a thyroid nodule.'
Reformatted CC4 and specified thyroid antibodies (Tg-Ab, TPOAb, TRAB, TSI) with coverage once every three years for individuals with hypothyroidism or hyperthyroidism.
