Anesthesia coverage criteria

Anesthesia services include pre/postoperative monitoring, administration of drugs/fluids, local infiltration and topical anesthesia; these elements are considered content of the anesthetic or surgical service and are not separately reimbursed.

Anesthesia time begins with initial administration of anesthetic agents by the anesthetist/anesthesiologist and ends when the anesthetist/anesthesiologist is no longer in personal attendance; anesthesia time and CPT anesthesia codes (total minutes) are required on all claims.

Payment calculation: Payment is determined by (CPT/ASA base units + time units) rounded up to the next whole anesthesia unit and multiplied by the BCBSKS anesthesia conversion factor; time units are calculated as one point per each 15 minutes.

Multiple surgical procedures: when one anesthesia provider covers multiple procedures, bill the CPT code with the highest base value; other procedures are paid per multiple procedure guidelines (additional procedures may be paid at one half), and anesthesia units are rounded up for payment.

Unusual case fees

Submit modifier 22 with supporting medical documentation to request unusual fee consideration; review process applies for determination.

Regional anesthesia: if a surgeon/assistant performs a regional block, payment may be allowed unless an anesthesia provider subsequently monitors the patient after the block (in which case the surgeon/assistant relinquishes the right to bill); epidural catheter placement may be paid or considered content depending on purpose (anesthesia vs postoperative pain management); daily management may be paid when applicable.

Obstetric epidural rules: epidural placement should be billed separately under the placement CPT code and placement time should NOT be included in monitoring/delivery anesthesia time; reimbursement for monitoring is one unit per hour for vaginal deliveries and one unit per 15 minutes for cesarean deliveries; anesthesia monitoring ends at delivery for vaginal and at transfer to PACU for cesarean.

Same-day nerve blocks: nerve blocks administered on the same day as an anesthesia service will be paid at 50% while the anesthesia service is paid in full.

Monitored anesthesia services (MAC)

Payment may be allowed when an anesthesia provider capable of initiating general anesthesia is required and documents that capability and necessity; documentation must support medical necessity and BCBSKS will monitor appropriate use.

Moderate (conscious) sedation: allowed and payable when medically necessary in inpatient or outpatient facility settings when performed by an authorized anesthesia provider (other than the provider of the primary service); moderate sedation in an office setting is considered content of the office procedure and will be denied as a provider write-off. Documentation must support medical necessity.

Medical direction/supervision: medical direction or supervision of CRNAs is not separately reimbursable; BCBSKS will reimburse only one provider for an anesthesia procedure.

Billing modifiers: all general anesthesia claims must include the appropriate anesthesia modifier (AA, AD, QK, QX, QY, QZ) in the first modifier position to identify the provider of anesthesia services.

Prior Authorization

Requesting an unusual case fee (modifier 22)

Use modifier 22 and submit supporting medical information that substantiates the unusual risk and documents direct attendance when requesting consideration of an unusual fee. Contracting providers agree to accept the review process determination for such requests.

Include modifier 22 on the claim.
Attach medical documentation substantiating the unusual patient risk and direct attendance.
Unusual detention may be eligible for additional time charges and should be documented.

Documentation Required

Document medical necessity for monitored anesthesia and moderate sedation

Ensure documentation clearly supports the medical necessity of monitored anesthesia services and medically necessary moderate (conscious) sedation. BCBSKS will monitor appropriate use and requires documentation to substantiate the anesthesia service and care provided.

For monitored anesthesia services (e.g., CT, MRI, cardiac cath/PTCA), document why anesthesia capable of initiating general anesthesia was required.
For moderate sedation in facility settings, document medical necessity and that the provider is authorized to administer general anesthesia under state law.
Moderate sedation performed in an office setting is considered part of the office procedure and will be denied as a provider write-off; document accordingly.
