Some multitarget PCR assays for bacterial vaginosis are marketed as laboratory-developed tests rather than cleared devices. Two assays are FDA-cleared (BD Max and Aptima BV) while others (including NuSwab VG, OneSwab BV Panel PCR with Lactobacillus Profiling, and SureSwab BV) are described as laboratory-developed tests and are not specifically FDA approved. Laboratories offering laboratory-developed tests must meet CLIA high-complexity requirements, and coverage decisions are subject to the policy stance and the member's contract benefits at the time of service.

Because the policy considers multitarget PCR testing for BV experimental/investigational, the FDA-clearance or LDT status of a specific assay is relevant to regulatory and laboratory practice but does not by itself establish medical necessity under this coverage policy.

Several diagnostic accuracy studies excluded or reported indeterminate reference-standard (Nugent) results. For example, Elvy et al excluded 46 indeterminate Nugent samples from the evaluable dataset, and other studies reported samples that were ineligible or not evaluable. Such exclusions reduce the evaluable sample set and can affect the applicability of reported test performance estimates.

The evidence base includes diagnostic accuracy studies and some studies that enrolled asymptomatic participants. However, no randomized controlled trials were identified that demonstrate improved health outcomes when multitarget PCR testing is used instead of standard diagnostic approaches. As a result, routine use of multitarget PCR for screening asymptomatic individuals or asserting superior clinical outcomes is not supported by direct evidence.

Guidance documents referenced in the evidence review (eg, CDC and ACOG summaries discussed in the policy) emphasize NAATs primarily for symptomatic women and note that multiplex PCR approaches remain under evaluation for clinical utility.

Standard diagnostic approaches include clinical assessment using Amsel criteria (requiring at least three of four clinical findings) and microscopic assessment using the Nugent Gram-stain score (with scores ≥7 consistent with BV and intermediate scores sometimes indeterminate). These comparators were used in the diagnostic accuracy studies reviewed.
