Defines coverage stance for measurement of plasma homocysteine for cardiovascular disease (CVD), venous thromboembolism (VTE), and other diagnoses, including medically necessary indications (homocystinuria, recurrent pregnancy loss, borderline B12 deficiency) and investigational/experimental determinations for screening/evaluation/management of CVD and VTE.
Merged two prior homocysteine policies into a single Homocysteine Testing policy.
Added medically necessary non-cardiac indications: recurrent pregnancy loss and venous thromboembolism.
