Coverage Criteria: Active vs Historical Malignancy
Status classification
Covered when the following classification rules are met:
Supported by documentation of ongoing treatments: surgery; chemotherapy; radiation; bone marrow or stem cell transplantation; hormonal or immunotherapy; adjuvant therapy (e.g., tamoxifen, anastrozole, leuprolide, bicalutamide).
Long-term surveillance or routine testing (e.g., PSA) to monitor for recurrence does not by itself render a historical malignancy active.
