Randomized trials that evaluated pulsed electromagnetic field (PEMF) added to physical therapy did not demonstrate an incremental benefit for pain reduction, stiffness, or disability compared with physical therapy alone. Available trials are heterogeneous, often small, and have short follow-up, so long-term benefit of PEMF as an adjuvant is not established.

Recent professional guidance does not support these modalities for osteoarthritis: the Osteoarthritis Research Society International (OARSI) strongly recommends against both electromagnetic therapy and neuromuscular electrical stimulation for knee, hip, and multijoint OA because of low-quality evidence and concerns about biological plausibility.

Systematic reviews and randomized controlled trials have produced mixed and inconsistent results, with methodological shortcomings and a high risk of bias across studies. Consequently, there is insufficient evidence that pulsed electrical or electromagnetic stimulation produces clinically meaningful improvements in function, stiffness, or quality of life for arthritis.

Because studies are generally small, heterogeneous, and at risk of bias, and because reported benefits are inconsistent or limited to short-term outcomes, these therapies may be considered not medically necessary for treatment of osteoarthritis or rheumatoid arthritis when used without stronger supporting evidence of clinically meaningful benefit.
