TB screening documentation

Provide tuberculosis (TB) screening documentation for patients naïve to biologic or targeted synthetic therapies: a negative TST or IGRA within 12 months prior to initiating therapy. If TB screening is positive, include evaluation to rule out active disease (e.g., chest x-ray); do not administer Olumiant for active TB. If latent TB is identified, treatment must be started before initiation.

TB test type: Tuberculin skin test (TST) or interferon-gamma release assay (IGRA).

Step Therapy

Preferred product trial requirement

Step therapy requires trials of the plan's preferred products prior to use of non-preferred Olumiant. For rheumatoid arthritis, initial approval requires inadequate response or intolerable adverse event to at least two preferred products (examples: Enbrel, adalimumab-aacf, Rinvoq, Simponi, Xeljanz/Xeljanz XR) or the specified alternative pathways noted in the criteria.

Preferred products include: adalimumab-aacf, Enbrel, Cosentyx, Otezla, Rinvoq, Simponi, Skyrizi, Otulfi (ustekinumab-aauz), Tremfya, and Xeljanz/Xeljanz XR.
RA-specific requirement: inadequate response to at least TWO of the preferred products (Enbrel, adalimumab-aacf, Rinvoq, Simponi, and Xeljanz/Xeljanz XR) OR meet the alternative criteria (clinical reason to avoid TNF inhibitors plus inadequate response to Rinvoq and Xeljanz).

Alopecia Areata - Criteria for initial approval

Authorization of 6 months may be granted for adult members for treatment of alopecia areata when ALL the following criteria are met:
