Coverage Criteria

Covered when the following criteria are met:

Allowed indications: The requested drug is being prescribed for pain associated with cancer, sickle cell disease, a terminal condition, or pain being managed through hospice or palliative care.

Opioid-use-safety and monitoring: The patient can safely take the requested dose based on their history of opioid use AND the patient has been evaluated and will be monitored regularly for development of opioid use disorder; for chronic pain the patient's pain must be reassessed in the first month after the initial prescription or any dose increase and every 3 months thereafter to ensure benefit outweighs risks.

Acute pain extension: The patient requires extended treatment beyond 7 days for moderate to severe acute pain where use of an opioid analgesic is appropriate.

Prioritize non-opioid treatments: Reserve opioid combination products for patients for whom alternative non-opioid analgesics have not been tolerated or have not provided adequate analgesia or are not expected to provide adequate analgesia.

Quantity limits for specific products: For hydrocodone/ibuprofen tablets: patient will not require more than 50 tablets per month. For tramadol/acetaminophen tablets: patient will not require more than 40 tablets per month.

Benefits and coverage under this policy depend on the member's contract and applicable exclusions or limitations. Benefit determinations are based on the applicable contract language in effect at the time services were rendered, and individual member benefits must be verified. This policy may not apply to Federal Employee Program (FEP) members; FEP benefits are determined by the Federal Employee Program and any exclusions, limitations, or exceptions in that contract.

Acute pain duration limits are waived when there is documentation that the patient is being treated for cancer, sickle cell disease, receiving palliative or hospice care, or has related claims or member-profile diagnoses within the prior 365 days. Evidence acceptable for this exemption includes relevant prescription claim history, ICD‑10 diagnosis codes for cancer, sickle cell disease, or palliative care, or a hospice patient residence code on the claim. Note that two product-specific exceptions remain: requests for hydrocodone/ibuprofen or tramadol/acetaminophen are still subject to maximum durations specified in their product labeling even when an exemption applies.

Long-term opioid therapy carries known risks and limited evidence of sustained benefit. Observational data link initial opioid exposure for acute pain to subsequent long-term use, and controlled trials show only small short-term improvements in pain with opioids and sparse or no evidence for benefit beyond months. Accordingly, continued therapy beyond defined approval periods should be re-evaluated and discontinued or tapered when benefits do not outweigh risks. When continuation is considered, clinicians should periodically reassess opioid‑related risk (including within 1–4 weeks after treatment initiation or dose escalation and at least every 3 months for long‑term therapy) and document that the expected benefits outweigh potential harms.
