This policy defines coverage, quantity limits, and accumulation rules for immediate-release opioid combination products (brand and generic) that contain non-opioid components such as acetaminophen, aspirin, or ibuprofen.

Coverage is provided without prior authorization for a 30-day or 90-day supply when the monthly quantity does not exceed the product's FDA-labeled maximum daily dosing, and when monthly quantities correspond to < 90 MME/day unless the product's FDA-labeled maximum strength/dosage frequency exceeds 90 MME/day.

All covered products must also stay within the maximum daily limits for non-opioid components: 4 g/day (4000 mg/day) acetaminophen or aspirin and 3200 mg/day ibuprofen.

Coverage Conditions

Coverage is provided without prior authorization when ALL of the following are met:

Immediate-release combination product: Prescription is for an immediate-release combination product opioid (includes non-opioid components acetaminophen, aspirin, or ibuprofen).
Supply requested is a 30-day or 90-day supply with monthly quantity not exceeding the maximum daily dose listed in product labeling.
Uses 25 days for 30-day fill and 75 days for 90-day fill to allow processing.
Monthly quantity limits: Monthly quantities correspond to < 90 MME/day and contain < 4 g/day acetaminophen or aspirin and < 3200 mg/day ibuprofen, unless the FDA-labeled maximum strength/dosage frequency exceeds 90 MME/day.MME < 90/day; APAP/ASA < 4 g/day; IBU < 3200 mg/day
Unless maximum FDA-labeled strength/dosage frequency exceeds 90 MME/day.

Quantity Limit Rejection

Claims will reject when ANY of the following are true:

Exceeds initial quantity: Patient is requesting more than the initial quantity limit (monthly or 3-month) specified for the drug/strength.
Claims will reject with a message indicating quantity limits are exceeded.
Exceeds cumulative limits: Requested quantity would exceed cumulative limits for the accumulation group (see accumulation rules) resulting in > 4 g/day APAP or ASA or > 3200 mg/day ibuprofen or > 90 MME/day (unless FDA-labeled max exceeds 90 MME/day).APAP/ASA > 4 g/day; IBU > 3200 mg/day; MME > 90/day

Quantity limits are set so monthly supplies do not exceed the product labeling daily maximums and generally correspond to < 90 MME/day, with an exception when the FDA-labeled maximum strength/dosage frequency exceeds 90 MME/day.

Because immediate-release opioid combination products include non-opioid components that have their own FDA-labeled daily maximums, the policy enforces additional safety limits of 4 g/day acetaminophen or aspirin and 3200 mg/day ibuprofen. These non-opioid component limits apply to all patients regardless of concomitant conditions (for example, cancer, sickle cell disease, palliative or end-of-life care).

Claims will reject if a patient requests more than the initial monthly or 3-month quantity limit or if accumulation across grouped products would exceed the APAP/ASA/ibuprofen or MME thresholds.

Initial Month Limit: Quantity allowed for a 30-day fill period (uses a 25-day duration for processing) aligned to daily dosing and MME/APAP/ASA/IBU limits.

Initial 3 Month Limit: Quantity allowed for a 90-day fill period (uses a 75-day duration for processing) aligned to daily dosing and MME/APAP/ASA/IBU limits; may be 'Does Not Apply' for drugs indicated only for short-term use.

Accumulation groups: Drugs accumulate into five groups for quantity limits: 1) APAP-containing solutions/suspensions/elixirs; 2) APAP-containing tablets/capsules; 3) Aspirin-containing tablets/capsules; 4) Ibuprofen-containing tablets; 5) Celecoxib-containing tablets.
