For individuals with cancer-related pain, the available evidence includes systematic reviews, randomized controlled trials, observational studies, and case studies that report improvements in some pain-related outcomes (e.g., pain rates, fatigue, sleep, mood) with biofeedback interventions; however, trials vary in quality, size, and outcomes measured and longer-term benefit and impact on net health outcome remain uncertain (see Spada et al. 2022; summary of evidence).
Evidence supports that biofeedback is safe and noninvasive; guideline support (NCCN) cites biofeedback as an optional intervention for cancer pain, which informs the decision to allow coverage when specific criteria are met.
For chronic non-cancer pain conditions (including, but not limited to, low back, knee, neck/shoulder, orofacial, abdominal pain, fibromyalgia, osteoarthritis, systemic lupus erythematosus, and vulvar vestibulitis), multiple RCTs and systematic reviews have not consistently demonstrated clinically meaningful or durable benefit of biofeedback compared with sham, usual care, or active conservative treatments; many trials are small, short in duration, and at moderate to high risk of bias. The evidence is insufficient to conclude improvement in net health outcomes for these conditions.
Systematic reviews (e.g., Cochrane reviews, Glombiewski et al., Campo et al.) and RCTs (e.g., Kapitza 2010; Yelden 2024; Lazaridou 2023; others) summarize heterogenous results with frequent null findings or benefits no greater than comparator treatments.
Randomized trials comparing biofeedback with sham or active interventions often show no significant between-group differences, and when differences are reported they are frequently small, of uncertain clinical significance, or not durable; addition of biofeedback to other active therapies (e.g., CBT, exercise) has generally not shown incremental benefit over the active therapy alone.
Examples include trials in low back pain, fibromyalgia, and shoulder/neck pain where biofeedback did not outperform sham or active comparators (see Kapitza 2010; van Santen 2002; Glombiewski et al. 2013).
Home-use biofeedback devices (including devices such as FreeSpira, Heart Rate Variability devices, InTandem, StressEraser) have no identified clinical-effectiveness studies; therefore, there is insufficient evidence to support their use for any indication.
