Correlative findings: Some studies show correlation between CDP parameters and other balance or functional measures (eg, walking velocity, Berg Balance Scale), but the clinical impact on management decisions and outcomes is uncertain.

Examples include Fritz et al. 2015 and Ferrazzoli et al. 2015; retrospective series report associations with falls but do not demonstrate altered management that improves outcomes.

Computerized dynamic posturography (CDP) is not appropriate for diagnosing pathology or for localizing the site of a lesion. The test quantifies an individual’s balance control under altered sensory conditions but does not provide the diagnostic specificity required to determine anatomic or pathologic causes of vestibular dysfunction. CDP is not a substitute for vestibular localization testing, and results should not be used in isolation to attribute symptoms to a specific vestibular lesion.

Use of dynamic posturography as a definitive diagnostic test for balance disorders is unsupported because there is no accepted criterion standard against which sensitivity and specificity can be measured. Systematic reviews and evidence summaries have not identified studies meeting inclusion criteria that establish clinical validity of CDP for diagnosing vestibular disorders, and comparative data demonstrating diagnostic accuracy are lacking.

Background and Context

Loss or impairment in any of these domains increases the risk of instability and falls, which are common and clinically consequential, particularly in older adults. CDP provides a quantitative assessment of how an individual uses sensory information for postural control by altering platform and visual surround conditions to isolate sensory contributions. However, because CDP does not identify specific pathologies and there is limited evidence that its use changes management to improve outcomes, its role is restricted to providing descriptive information about balance function rather than definitive diagnostic or prognostic conclusions.
