Testing that does not meet the policy’s criteria for establishing a diagnosis of inflammatory bowel disease (IBD), for guiding management of active IBD, or for detecting relapse is not supported because current evidence is insufficient to demonstrate an improvement in net health outcomes.

Fecal biomarker testing is not appropriate when there is already an indication for immediate endoscopy. Specifically, patients with alarm features (for example, rectal bleeding, advanced age or other risk factors for colorectal cancer) or when alternative causes of symptoms have not been excluded should proceed to endoscopic evaluation rather than relying on fecal calprotectin or lactoferrin to triage care. The primary intended use for fecal testing in this context is to inform the decision whether to proceed to endoscopy when endoscopy with biopsy is being considered and no other immediate indications exist.

Several diagnostic studies used for pooled analyses excluded research of fecal calprotectin when the intent was to monitor disease progression or treatment response. For example, the Waugh et al. (2013) systematic review excluded studies that used fecal calprotectin tests to monitor progression or response to therapy, reflecting that the body of evidence supporting monitoring differs from diagnostic study designs and was not pooled in that review.

Clinical utility for using fecal calprotectin to monitor active IBD has not been established. Because a chain of evidence linking test performance to improved patient outcomes (preferably from randomized trials) is lacking, conclusions about the usefulness of fecal calprotectin to guide management of active disease remain uncertain.

Routine use of fecal calprotectin to direct management decisions in active IBD is not supported in the absence of corroborating clinical or endoscopic findings. The evidence for monitoring and for test-guided changes in therapy is inconsistent, and routine reliance on fecal calprotectin alone to make definitive management changes may therefore be excluded from coverage.

Fecal lactoferrin is a stool marker of neutrophil-mediated intestinal inflammation but has limitations. Lactoferrin reflects local gut inflammation and can be elevated in conditions other than IBD (for example, colorectal neoplasia or polyps). While pooled diagnostic analyses report reasonable sensitivity and specificity, the clinical utility of fecal lactoferrin for monitoring or as a standalone diagnostic replacement for endoscopy remains uncertain.

Clinical guidance excludes fecal calprotectin testing in settings where cancer is suspected. NICE specifically recommends fecal calprotectin only when cancer is not suspected and specialist assessment is being considered; if malignancy is a concern, colonoscopic evaluation is required rather than biomarker-based triage.

Fecal biomarker testing should not be used to evaluate patients with acute bloody diarrhea or older patients in whom colonoscopy is needed to rule out polyps or cancer. Use of fecal calprotectin or lactoferrin in these contexts could delay necessary diagnostic endoscopy and is therefore inappropriate.

Fecal calprotectin may be unreliable in patients with isolated small-bowel disease, particularly pure ileal Crohn disease. In such cases a negative fecal calprotectin result should not be used to exclude disease, and endoscopy or imaging studies may still be required because stool biomarkers may not reflect isolated small-bowel inflammation.

Testing for indications beyond diagnosing IBD, managing active disease, or monitoring for relapse is considered investigational. Uses that are not explicitly supported by the policy’s covered indications lack sufficient evidence to demonstrate a net clinical benefit.

Fecal calprotectin and lactoferrin are adjunctive tests and are not definitive substitutes for endoscopy when endoscopic evaluation is indicated. Although fecal calprotectin has high sensitivity at commonly used cutoffs (for example, 50 µg/g), a negative stool biomarker does not obviate endoscopy in the presence of alarm features or high pre-test probability of disease.

There are no randomized controlled trials demonstrating that management strategies guided solely by fecal calprotectin testing improve patient-level health outcomes. The absence of direct RCT evidence weakens the ability to conclude that test-guided management reduces relapse rates, morbidity, or improves quality of life.

Management decisions should not be based solely on fecal calprotectin in the absence of supporting evidence that such an approach improves outcomes. Because clinical utility for monitoring active IBD has not been established, clinicians should integrate fecal calprotectin results with symptoms, serum markers, and, when indicated, endoscopic findings before changing therapy.

Using fecal calprotectin alone to reduce routine surveillance endoscopy or to make definitive long-term management changes is limited by inconsistent evidence and lack of high-quality trials. Existing RCTs are few, small, or have methodological limitations, so reliance on FCP alone to decrease surveillance endoscopy frequency is not supported without corroborating clinical data.

Fecal lactoferrin should not be used as a standalone replacement for endoscopy. While meta-analyses report moderate-to-high pooled sensitivity and specificity, the clinical utility of lactoferrin to safely defer endoscopy in patients with indications for endoscopy has not been established by randomized trials.

Biomarker elevations without clinical corroboration can lead to overtreatment. The AGA warns that overtreatment based solely on biomarker results—when symptoms are due to noninflammatory (functional) causes—can increase adverse effects without symptomatic benefit; therefore, biomarker results should prompt confirmatory evaluation rather than automatic escalation.

Fecal calprotectin alone should not replace endoscopy when pre-test probability, clinical features, or supporting laboratory findings indicate a high likelihood of disease. Decision to defer endoscopy based on FCP requires documented pre-test probability, assay cutoff used, and supporting clinical data; otherwise, direct endoscopic evaluation may still be necessary.
