Coverage Criteria for Omnipod Disposable Insulin Delivery Systems

Initial Therapy

Covered when ALL of the following are met:

Initial Approval Criteria: 1) Prescription is by or in consultation with an endocrinologist or a provider with Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) certification; 2) Member has utilized at least one of the following insulin administration methods for ≥6 months: use of an insulin pump OR administration of at least 3 daily injections of a basal and bolus insulin regimen (multiple daily injections); 3) History of suboptimal blood sugar control despite appropriate management, examples include: episodes of diabetic ketoacidosis (DKA); repeated hypoglycemic events (blood glucose <70 mg/dL); wide blood glucose fluctuations; hypoglycemia unawareness; dawn phenomenon with fasting blood glucoses repeatedly >200 mg/dL; or HbA1c ≥7.0; 4) Member has monitored blood glucose ≥4 times per day for at least the last 6 months (or equivalent CGM documentation per clinical judgment); 5) Member or caregiver has completed physician-directed comprehensive diabetes management education; 6) Requested number of pods does not exceed 10 per month (additional pods may be approved if member requires >200 units of insulin every 3 days).see text

Continuation Therapy

Continuation of therapy is approved when ALL of the following are met:

Continuation Criteria: 1) Member is experiencing clinical benefit from the Omnipod disposable insulin delivery system (for example, improved glucose control or better adherence to therapy); AND 2) Member continues to meet the initial approval criteria (including ongoing monitoring, diabetes education, and insulin administration requirements).see text

Quantity Limits / Supply

Device supply limits: Starter kit contains the personal diabetes manager (PDM) and 5 pods; starter kit limited to one per 5 years. Per the quantity limits, a starter kit (which includes the personal diabetes manager and 5 pods) is limited to one kit every 5 years, and pods are generally limited to 10 pods per 30 days. The starter kit frequency and pod allowance are enforced through the prior authorization process; requests exceeding these limits will be subject to post‑limit prior authorization review as described in the policy. Note that Omnipod DASH intro kits are not covered under the standard benefit allowance.

Requests that fail to meet the initial approval criteria will be considered not medically necessary. Common examples include: insufficient documented prior insulin administration (the member must have used either an insulin pump or at least 3 daily injections of basal and bolus insulin for the last 6 months), inadequate frequency of self‑monitoring of blood glucose (fewer than 4 checks per day for the prior 6 months without documented CGM use), lack of documented physician‑directed comprehensive diabetes management education, or requested pod quantities that exceed policy limits without justification. Also, requests lacking required prescribing or consultation by an endocrinologist or a provider with CDCES/CDE certification may be denied.
