Contrave (naltrexone-bupropion)

Covered when ALL of the following are met:

Documentation required for renewal.

Drugs for weight loss management are a standard exclusion for most benefit plans. When a member's prescription drug benefit explicitly includes coverage for weight loss products, coverage determinations are made only if the benefit exists and are applied consistent with the member's contract. Prior authorization is required and medical necessity will be evaluated against product labeling and the policy's agent-specific criteria.

Requests for Wegovy or Zepbound when the reason for use is type 2 diabetes mellitus as a comorbid, weight‑related condition are excluded from coverage. Such requests should be resubmitted for a GLP‑1 receptor agonist or a GIP‑GLP‑1 receptor agonist that is FDA‑approved for patients with type 2 diabetes mellitus.

The requested agent must not be used in combination with another targeted weight‑loss agent for the same indication. Specifically, coadministration with fenfluramine (Fintepla) is prohibited and combination use with any other labeled weight‑loss pharmacologic product for the requested indication is not allowed.

Members who receive medication as samples or through a manufacturer's patient assistance program must meet the policy's criteria for initial approval. Access to samples or assistance does not bypass prior authorization requirements or substitute for the documentation required to demonstrate medical necessity.

Coadministration of agents within the same molecular class is not recommended. Examples include liraglutide‑containing products not to be coadministered with other liraglutide or GLP‑1 receptor agonists, semaglutide‑containing products not to be coadministered with other GLP‑1 receptor agonists, and tirzepatide‑containing products not to be coadministered with other tirzepatide or GLP‑1 receptor agonists.

Drugs addressed in this policy are considered not medically necessary when the member does not meet the agent‑specific criteria set forth in the policy (including age, BMI thresholds, prior program participation, trial duration and documented response). Denials may result when required documentation or response thresholds are not provided.

For Saxenda (liraglutide), the safety and effectiveness in pediatric patients with type 2 diabetes mellitus have not been established. Saxenda should not be used in pediatric patients with type 2 diabetes unless supported by appropriate evidence and documentation, as noted in the product labeling.
