Covered when ALL of the following groups and their listed conditions are met:

  - When history, signs or symptoms suggest use of non-prescribed medications or illegal substances

  - Monitoring up to four times per year after initiation of therapy
- Testing at time of visit for individuals with aberrant behaviors (lost prescriptions, multiple requests for early refills, opioids from multiple providers, unauthorized dose escalation, apparent intoxication)
- Testing when there is a history of maternal substance abuse or agitated/altered mental status in the birthing parent
- Testing for pregnant individuals at high risk for substance abuse when suspicion exists per screening questions or PDMP information, as documented
- Assessment of organ transplant candidates with substance abuse history; individuals with suspected or diagnosed mental illness, ADHD/disruptive disorders, epilepsy, and cancer patients on opioid therapy; management/monitoring in substance abuse treatment per specified frequencies

Definitive (confirmatory) drug testing (qualitative or quantitative) may be reimbursable for up to seven drug classes when a laboratory-based definitive test is specifically requested, the ordering clinician documents the medical rationale in the individual's record, and ANY of the following apply: the presumptive screen result is inconsistent with the individual's medical history, clinical presentation, or patient statement (for example, a negative result for prescribed medication or a positive result for an unprescribed/illegal drug); diagnostic or monitoring needs for substance use disorder where accurate and reliable results are required for treatment decisions; the presumptive test does not exist or cannot adequately detect the specific drug or metabolite; or other clinical circumstances documented by the treating clinician (e.g., blood specimen required for patients with anuric chronic renal failure). The ordering clinician must specifically request definitive testing in the written order and document the rationale in the medical record.

Orders and documentation — Required elements and limits

Maintain complete, patient-specific documentation that justifies each drug test ordered. The medical record must include relevant medical history, physical exam, results of prior diagnostic tests, and the clinician's written order naming each specific drug or drug class to be tested. Requisitions must identify patient name and DOB; specimen type; who collected the sample; facility/location of collection; date/time received by the laboratory; and a legible dated signature (electronic or handwritten) of the ordering clinician. Panel references, 'custom panel', reflex/automatic testing, or blanket/standing orders are not acceptable.
