Scope: This policy addresses coverage and reimbursement determinations for laboratory testing related to the diagnosis, assessment, prognosis, and severity determination of acute pancreatitis, including serum lipase, serum amylase, urinary amylase, trypsin/trypsinogen/TAP, and selected inflammatory biomarkers.
Applies to Blue Cross Blue Shield - Illinois members and participating providers.
Clinical context: For individuals presenting with signs and symptoms of acute pancreatitis, certain tests may be reimbursable (see reimbursement criteria).
Signs and symptoms include epigastric pain, nausea, vomiting, fever, tachycardia, hypotension, and other findings listed in NOTE 1.

Reimbursement criteria

Reimbursement determinations for laboratory tests related to acute pancreatitis.

ALL of the following

Measurement of serum lipase may be reimbursable for individuals presenting with signs and symptoms of acute pancreatitis (see NOTE 1).
Reimbursable only when ordered for initial evaluation of suspected acute pancreatitis.
Not reimbursable — serum lipase
- For individuals with an established diagnosis of acute or chronic pancreatitis.
- More than once per visit.
- For asymptomatic individuals during a general exam without abnormal findings.

Documentation Required

Documentation, coding, and claims submission requirements

Providers must submit accurate documentation of services performed and submit claims using valid code combinations from HIPAA‑approved code sets. Claims should be coded according to industry standard coding guidelines (for example, Uniform Billing Editor; American Medical Association CPT and CPT® Assistant; HCPCS; ICD‑10‑CM/PCS; NDCs; DRG guidelines; CMS National Correct Coding Initiative Policy Manual and CCI table edits). Claims are subject to code edit protocols and to review against plan documents, provider contract language, medical policies, clinical payment and coding policies, and coding software logic; upon request, providers must submit any additional documentation.

Submit claims with valid HIPAA‑approved code combinations and appropriate coding per industry standards.
Be prepared to provide additional documentation upon request to support medical necessity and coding.
Claims may be denied or adjusted if coding or documentation do not comply with plan documents, provider contracts, medical policies, or code edit protocols.

NOTE 1 — Signs and symptoms of acute pancreatitis

Definition (NOTE 1)Mild to severe epigastric pain that begins slowly or suddenly (may spread to the back in some patients).

Associated symptomsNausea, vomiting, anorexia, diarrhea; abdominal distention; tender to palpation of epigastrium.

Systemic signsFever, rapid pulse, tachypnea, hypoxemia, hypotension.

Physical findingsHypoactive bowel sounds, Cullen sign, Grey Turner sign.

When ordered for anything other than analysis of pancreatic cyst fluid, measurement of serum amylase is not reimbursable.

Serum amylase reimbursement limited to pancreatic cyst fluid analysis indications only.

Measurement of serum and urine trypsin/trypsinogen/TAP (trypsinogen activation peptide) for diagnosis, assessment, prognosis, or determination of severity of acute pancreatitis is not reimbursable.

Includes both serum and urine assays of these analytes.

Measurement of the following biomarkers for diagnosis, assessment, prognosis, or determination of severity of acute pancreatitis is not reimbursable: C-Reactive Protein (CRP), Interleukin-6 (IL-6), Interleukin-8 (IL-8), Procalcitonin.

These inflammatory markers are considered nonreimbursable for these indications.

Measurement of urinary amylase concentration for the initial diagnosis of acute pancreatitis is not reimbursable.

Urinary amylase not covered for initial diagnostic use.

For all other situations or conditions not described above, measurement of serum lipase is not reimbursable.

Default noncoverage for serum lipase outside the specified, symptomatic initial-evaluation context.
