Coverage Criteria for Iron-Related Laboratory Testing

Reimbursement criteria for iron-related lab testing

Covered when any of the following conditions are met; tests and uses specifically listed are not reimbursable.

ANY of the following

ALL of the following
- For the evaluation of an individual with abnormal hemoglobin and/or hematocrit levels
- For the evaluation and monitoring of iron overload disorders
- For individuals with first-degree relatives with confirmed hereditary hemochromatosis (HH)
- For individuals with symptoms of hemochromatosis
- For the evaluation of individuals with liver disease
- For the evaluation of hemophagocytic lymphohistiocytosis (HLH) and Still Disease
- In males with secondary hypogonadism
- For the evaluation and monitoring of patients with chronic kidney disease who are receiving or being considered for receiving treatment for anemia
- At a frequency of every 3 months for individuals on iron therapy

ANY of the following

ALL of the following
- For the evaluation of iron overload in individuals with symptoms of hemochromatosis
- For the evaluation of iron overload in individuals with first-degree relatives with confirmed hereditary hemochromatosis (HH)
- For the evaluation of iron deficiency anemia

ALL of the following

Measurement of ferritin or transferrin (including transferrin saturation) in situations not addressed above is not reimbursable
Serum hepcidin testing, including immunoassays, is not reimbursable
Use of GlycA testing to measure or monitor transferrin or other glycosylated proteins is not reimbursable
Note: carbohydrate-deficient transferrin is out of scope for this policy

Providers are responsible for submission of accurate documentation of services performed and must code claims using valid HIPAA code sets and industry coding guidelines. Upon request, the provider is urged to submit any additional documentation to support the claim.

Submit claims using valid HIPAA-approved code sets (CPT, HCPCS, ICD-10, etc.).
Code appropriately per industry standards (Uniform Billing Editor, AMA CPT Assistant, CMS NCCI, DRG guidelines, etc.).
Provide additional documentation when requested to support medical necessity and coding.
