Coverage Criteria

Coverage is conditional. Specific reimbursable and non-reimbursable situations are described below.

ALL of the following

ANY of the following
- Individual has atypical clinical, laboratory, or imaging features.
- Individual has an atypical clinically isolated syndrome (including but not limited to primary progressive MS or atypical relapsing-remitting course).
- Individual belongs to a population in which MS is less common (e.g., children, older individuals).
- Individual has insufficient clinical or imaging evidence for diagnosis.
ALL of the following
- The individual has monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof.
- The individual has radiological or electrophysiological findings compatible with central nervous system (CNS) demyelination.
- ANY of the following
  - Belongs to a higher‑risk population (e.g., pediatric).
  - Abnormal MRI depicting extensive optic nerve lesion, extensive spinal cord lesion or atrophy, or large confluent T2 brain lesions.
  - Prominent papilledema, papillitis, or optic disc swelling during acute optic neuritis.
  - Neutrophilic CSF pleocytosis.
In all other situations, serum biomarker tests for multiple sclerosis are not reimbursable.
Operational: Claims for serum MS biomarker testing outside the situations above should be denied as not reimbursable.
ELISA, Western blot, immunohistochemistry, or any other non‑specified serum assays to test for NMOSD or MOG‑EM are not reimbursable.
Operational: Reimburse only AQP4‑IgG and MOG‑IgG by validated indirect fluorescence assay or FACS when criteria above are met.
For the diagnosis of MS, NMOSD, or MOG‑EM, all other cerebrospinal fluid (CSF) biomarker tests, including CSF AQP4‑IgG or CSF MOG‑IgG, are not reimbursable.
Operational: CSF testing for AQP4‑IgG or MOG‑IgG should not be billed for reimbursement.

Not all lab management reimbursement requirements apply to every product—review plan documents for eligibility.

Oligoclonal band analysis

Reimbursable indication (CSF/serum OCB)Oligoclonal band analysis (CSF and serum) may be reimbursable for diagnosis of MS in individuals with atypical clinical, laboratory, or imaging features.

Clinical presentationsReimbursable when individual has monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination.

Supporting findings requiredReimbursable when radiological or electrophysiological findings are compatible with CNS demyelination.

High-risk featuresAt least one additional high-risk feature required (examples: pediatric age, extensive optic nerve or spinal cord lesion, prominent optic disc swelling, neutrophilic CSF pleocytosis, histopathology with intralesional complement/IgG or prior 'pattern II MS', bilateral acute optic neuritis, severe visual deficit, severe/frequent myelitis or brainstem encephalitis, permanent sphincter/erectile dysfunction after myelitis, or prior ADEM).
