ABA coverage and billing criteria

Coverage is subject to member benefits and the following payer rules and documentation requirements.

Provider qualification: ABA services reimbursable only when provided by a Qualified Healthcare Professional (QHP) who is BACB-certified as a Behavior Analyst and/or state-licensed as a Behavior Analyst or Licensed Psychologist.

Non-covered purposes and settings: Services provided for educational, vocational, respite, or custodial purposes are not reimbursable. Programs/services rendered in non‑conventional settings (anything other than POS codes 10, 11, 12) require supporting documentation in the member record and rationale for any unusual POS to be considered for reimbursement.

Assessment and evaluation limits: Treatment plans and/or evaluations (including administration, scoring, interpretation, and report write‑up) that exceed eight hours (32 units of CPT 97151) may not be eligible for reimbursement. CPT 97151 cannot be reported concurrently with other codes.

Parent education authorization: Parent/caregiver education is authorized up to one hour per week for the authorization period (typically 26 weeks) for a total of 26 hours; requests for greater than one hour per week must include supporting clinical documentation.

Claims must be coded per industry-standard coding guidelines and supported by documentation. CMS Medically Unlikely Edits (MUE) indicate direct ABA services are typically requested for up to approximately 40 hours per week; please refer to the most current CMS MUE table for guidance on maximum units of service per single member on a single date of service. Service units are also constrained by the member's specific authorization period. Documentation must justify any units billed beyond industry standard coding guidelines or the CMS MUE guidance.

Ensure billed units align with the member's authorization period and any plan/state mandates.
Include justification and clinical documentation for any units exceeding industry-standard guidance or MUE limits.

Documentation Required

Documentation and claim substantiation

Providers must ensure documentation substantiates that services were rendered and supports the claim. Required documentation includes a parent/caregiver signature for each rendered service (including service/code, rendering provider name/signature, certification and credentials, place of service, date of service, and begin/end times), and a written account/summary/note of the service rendered. Unusual Place of Service (POS) codes or services rendered in non-conventional settings (other than POS 10, 11, 12) require rationale and supporting documentation in the member record.

Documentation must be available upon request and should support any unusual POS or non-conventional setting.
Assessment/evaluation time (including administration, scoring, interpretation, and report write-up) exceeding eight hours (32 units of 97151) may not be eligible for reimbursement per industry standard coding guidelines — provide documentation if exceeding.
Family Adaptive Behavior Treatment Guidance (CPT 97156) is for face-to-face caregiver/guardian guidance (with or without the patient present) and should be billed when that activity occurs; 97155 is reserved for supervisory activities related to the patient’s care.
Billable supervision must be face-to-face and involve only one technician; there is no CPT code for indirect (patient not present) supervision except 97151 and 97156 where permitted.

Qualified Healthcare Professional (QHP)

DefinitionQualified Healthcare Professional (QHP) — a provider certified by the Behavior Analyst Certification Board (BACB) as a Behavior Analyst and/or licensed in their state as a Licensed Behavior Analyst or Licensed Psychologist; only QHPs may be reimbursed for ABA services.

Reimbursement restrictionABA services are not reimbursable if provided by individuals who are not QHPs; unlicensed or non-qualified providers cannot bill through another person's NPI to receive Plan reimbursement.

Supervision noteServices performed by supervisees and billed through a supervisor must follow applicable state laws, Board requirements, and CMS conventions for supervisor-supervisee documentation and billing.

8-minute rule (timed service unit billing)

Rule definition8-minute rule — a timed service billing requirement: a timed service must be at least 8 minutes within the Medicare 15-minute unit ranges to count as one billable unit; apply the rule when multiple timed services occur on the same day.
