Outpatient Biomarker Reimbursement Criteria

Outpatient reimbursement criteria and exclusions for cardiac biomarkers:

Quantitative troponin for suspected ACS

Reimbursable up to four times within the first 72 hours following initial presentation.

Troponin qualitative exclusion: Qualitative measurement of cardiac troponin (troponin T or I) in the outpatient setting is not reimbursable.

BNP/NT-proBNP use

For chronic heart failure, reimbursable up to four times per year in the outpatient setting.

Older biomarkers exclusion: The following biomarkers are not reimbursable for diagnosis and/or prognosis of myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome: Aspartate aminotransferase (AST/SGOT), creatine kinase (CK), cardiac creatine kinase isoenzyme MB (CK-MB), creatine kinase isoenzymes, lactate dehydrogenase (LD/LDH), and myoglobin.

Specific excluded biomarkers: The following biomarkers are not reimbursable for diagnosis and/or prognosis of myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome: copeptin, troponin C, C-reactive protein, heart-type fatty acid binding protein (H-FABP), and any other cardiac biomarkers not listed as reimbursable.

ST2 exclusion: For all situations in the outpatient setting, analysis of ST2 and/or its isoforms (e.g., Presage ST2) is not reimbursable.

Outpatient facility capability requirement: Measurement of cardiac biomarkers for suspected acute coronary syndrome performed in an outpatient setting that is not capable of performing an adequate clinical MI evaluation (e.g., independent laboratory or physician's office) is not reimbursable.

Plan and coding expectations: If conflict exists between this policy and any plan document or provider contract, the plan document or provider contract governs; providers must submit accurate documentation and use valid HIPAA-approved code sets and industry coding guidelines; claims are subject to review.

Documentation and Claims Review — Providers responsible for submission of accurate documentation; may be asked to supply additional documentation upon request.

Providers are responsible for submission of accurate documentation of services performed. Providers are expected to submit claims for services rendered using valid code combinations from HIPAA-approved code sets and to code claims appropriately according to industry-standard coding guidelines (including, but not limited to, Uniform Billing Editor, AMA CPT, CPT® Assistant, HCPCS, ICD-10-CM/PCS, NDCs, DRG guidelines, CMS NCCI Policy Manual, and CCI table edits). Claims are subject to code edit protocols and claim review, which may consider benefit coverage, provider contract terms, medical policies, clinical payment and coding policies, and coding software logic. Upon request, providers must submit additional documentation to support the claim.

Typical ischemic chest painIschemic chest pain with radiation to an upper extremity, radiation to both arms, and pain associated with diaphoresis or with nausea and vomiting.

Descriptive chest sensationsSqueezing, tightness, pressure, constriction, crushing, strangling, burning, heartburn, fullness in the chest, band-like sensation, knot in the center of the chest, lump in throat, ache, heavy weight on chest and toothache (with radiation to the lower jaw).

Associated symptomsShortness of breath, belching, nausea, indigestion, vomiting, diaphoresis, dizziness, lightheadedness, clamminess, and fatigue.
