ALL of the following

Applies to individuals newborn through age 18 (excluding newborn genetic disorder screening).
Newborn screening panel may be reimbursable when it follows all applicable federal and state law recommendations.
Screening for hyperbilirubinemia in all newborns may be reimbursable.
Screening for congenital hypothyroidism in all newborns utilizing serum T4 and/or TSH may be reimbursable.
Screening for sickle cell disease in all newborns may be reimbursable.

Blood lead screening for individuals at increased risk for lead exposure (see Note 1) with frequency: one test at 6, 9, and 12 months; annually from age 2 through 6 years.Tests at 6, 9, 12 months; annually from 2–6 years
Anemia screening with hemoglobin or hematocrit: at 12 months for all individuals; for individuals 4 months and older if at risk for iron deficiency (see Note 2).12 months; 4 months if at risk
Tuberculosis screening for individuals 1 month of age or older who are at increased risk for TB (see Note 3).
Dyslipidemia screening with fasting lipid profile or non-fasting non-HDL-C: annually for children/adolescents at increased risk (personal or family history per Note 4); and once for all children during each age period including ages 9-11 years.One screen during ages 9–11 years

Providers must submit accurate documentation of services performed and claim submissions using valid HIPAA-approved code sets following industry coding guidelines. Upon request providers should submit additional documentation.

This policy defines reimbursement criteria for laboratory-based preventive screening tests for individuals from newborn through age 18 enrolled in Blue Cross Blue Shield of Illinois plans. Coverage is available with specific criteria and not all requirements apply to every product; providers should review plan documents for product-specific eligibility and prior authorization rules. The policy explicitly excludes newborn screening for genetic disorders from the scope of reimbursable services described here. Effective date: January 1, 2026 (Policy CPCPLAB016, Version 1.0).

Definitions

AdolescentAs defined by WHO, any person between age 10 and 19.

Increased risk for lead exposureCDC-defined factors: living or spending time in housing built before 1978; growing up in a low-income household; recent immigrant, refugee, or recently adopted from less developed countries; living or spending time with a person who works with lead or has lead-exposing hobbies.

Iron deficiency risk factors (children)AAP-defined factors: history of prematurity or low birth weight; lead exposure; exclusive breastfeeding beyond 4 months without supplemental iron; weaning to whole milk or complementary foods lacking iron-fortified or iron-rich foods; feeding problems; poor growth; inadequate nutrition.

TB risk factors (children)AAP-defined factors: close contact with a person with or suspected to have infectious TB; radiographic or clinical findings suggestive of TB; HIV infection or at risk for HIV; foreign birth/refugee/immigrant from high-TB areas; contact with high-risk populations (eg, homeless, incarcerated, illicit drug users, migrant workers); immunosuppression; living in or travel to high-risk/endemic areas.

Dyslipidemia risk factors (children)AAP-defined factors: family history of CVD or dyslipidemia (male relatives ≤55 years, female ≤65 years); unknown family history; overweight (BMI ≥85th to <95th percentile); obesity (BMI ≥95th percentile); hypertension (BP ≥95th percentile); cigarette smoking; diabetes mellitus.
