Average-risk screening: For average-risk individuals 45 years of age and older, screening with total PSA may be reimbursable.age >=45

Not reimbursable when ANY of the following apply:

First-line percent free PSA screening: Percent free PSA as a first-line screening test for prostate cancer is not reimbursable.

Routine screening using percent/free-to-total/complexed PSA: Percent free PSA, free-to-total PSA ratio, and/or complexed PSA tests for routine screening of prostate cancer are not reimbursable.

Percent free prostate-specific antigen (percent free PSA) tests are not reimbursable when used as a first-line screening modality for prostate cancer. Additionally, percent free PSA, the free-to-total PSA ratio, and complexed PSA assays are not reimbursable when performed for routine population screening of prostate cancer.

Routine ordering of percent free PSA, the free-to-total PSA ratio, or complexed PSA for asymptomatic screening is explicitly excluded from coverage and considered not medically necessary. These tests may not be submitted as substitutes for initial total PSA screening; reimbursement is limited to situations defined elsewhere in the policy (for example, short-interval follow-up or diagnostic evaluation after an elevated total PSA or suspicious digital rectal exam).

Coverage under this policy may vary by product and specific plan documents. If a conflict arises between this Clinical Payment and Coding Policy and any plan document under which a member is entitled to Covered Services, the plan document will govern. If a conflict arises between this CPCP and any provider contract pursuant to which a provider participates in and/or provides Covered Services to eligible member(s) and/or plans, the provider contract will govern. "Plan documents" include, but are not limited to, Certificates of Health Care Benefits, benefit booklets, Summary Plan Descriptions, and other coverage documents. Blue Cross and Blue Shield of Illinois (BCBSIL) may use reasonable discretion interpreting and applying this policy to services being delivered in a particular case. BCBSIL has full and final discretionary authority for their interpretation and application to the extent provided under any applicable plan documents.

Documentation Required

Documentation submission

Providers are responsible for submission of accurate documentation of services performed. Upon request, the provider should submit any additional documentation to support medical necessity, coding, and reimbursement. Providers must retain and produce clinical records, test results, reports, and any other supporting materials as required by the plan or BCBSIL.

Providers must submit complete and accurate documentation when requesting reimbursement or upon BCBSIL request.
Additional clinical information may be requested to validate medical necessity or to resolve claim reviews.

Billing Rule

Coding and claim submission requirements

Claims must be submitted using valid code combinations from HIPAA-approved code sets and coded according to industry-standard coding guidelines. Claims are subject to code edit protocols and claim review, which may include review against benefit coverage, provider contract language, medical policies, clinical payment and coding policies, and coding software logic. Claims may be denied or adjusted if coding or claim submissions do not follow applicable code sets or industry rules.

Use HIPAA-approved code sets: CPT, HCPCS, ICD-10-CM/PCS, NDCs, and other applicable code systems.
Follow industry coding resources and guidance (e.g., AMA CPT, CPT Assistant, UB, CMS NCCI policy manual, CCI edits).
Claims are subject to BCBSIL code edit protocols and may be reviewed or adjusted; improper coding may result in denial or adjustment.
