Covered when ALL of the following are met:
Prior authorization may still be required; providers must document trials of preferred agents as applicable.
Members who have received treatment with a non-preferred Part B drug within the past 365 days are exempt from the step therapy requirement. This lookback exemption means that if the member was administered the same non-preferred agent during the prior 365-day period, the requirement to first trial a preferred Part B drug does not apply.
