Concomitant use of oral medications for erectile dysfunction is NOT a covered benefit; previous therapies may be verified through pharmacy paid claims or submitted chart notes. Use of drug samples to initiate therapy does not meet step edit/preauthorization criteria.
Concomitant use of oral medications for erectile dysfunction is NOT a covered benefit. Requests that indicate concurrent use of more than one oral PDE5 inhibitor will not meet coverage criteria and should not be submitted for authorization.
