Unbundling reimbursement criteria

When a component procedure code is billed with a comprehensive procedure or in other disallowed combinations, the following reimbursement principles apply.

When a coding combination is identified where a comprehensive code includes component services, only the comprehensive code is reimbursable and the component code is not separately reimbursed.

Denial recommendations

A more comprehensive procedure is billed with a component procedure (component services are subsumed by the comprehensive procedure).
A procedure results in overlapping services with another billed procedure.
Procedures that are impossible to perform together during the same operative session (mutually exclusive procedures).
An evaluation and management (E/M) service is billed on the same date as a surgical procedure (subject to modifier considerations).

Modifier -25 may support separate reimbursement when the E/M represents a significant, separately identifiable service (e.g., initial visit for the condition, separate diagnosis treated, worsening condition requiring additional diagnostic procedures, or unrelated supplies/equipment necessitating E/M).

Modifier -57 indicates decision for surgery (day before or day of major surgery); E/M lines with -57 are subject to prepayment clinical validation and will be reimbursed only if documentation supports timing of the decision for surgery.

Modifier -59 and distinct procedural documentation: Modifier -59 is used to indicate distinct procedures/services on the same day; documentation should include diagnosis codes, claim history showing multiple sites, and applicable anatomical or site-specific modifiers to avoid denials.

Site-specific modifiers (for example, left/right or other site designations) are considered when evaluating bundling and distinct procedures.

Editing software and coding guidance: The health plan uses automated claims code editing software and specialty society coding rules (distinct from CMS NCCI Column 1/Column 2 tables) to identify unbundling relationships and make reimbursement determinations; providers should reference current professional coding guidance prior to claim submission.

Policy scope and applicability statements

This payment policy is a guide to payment and does not constitute a contract or guarantee of payment; coverage and payment determinations are subject to the terms of the member's plan documents, state and federal requirements, and applicable plan-level administrative policies and procedures.

Health Plan retains the right to change, amend or withdraw this payment policy; the posting date may not equal the effective date and applicable legal or regulatory requirements govern if discrepant.

For Medicaid members, when state Medicaid coverage provisions conflict with this payment policy, state Medicaid provisions take precedence.

For Medicare members, applicable Medicare National Coverage Determinations (NCDs) and Local Coverage Determinations (LCDs) should be reviewed prior to applying this policy's criteria.

Providers are expected to exercise professional medical judgment; submission of claims binds providers to the terms and conditions of this policy and related contracts.

The health plan evaluates claims using automated code-editing software. Claim service lines billed with a procedure code that is not separately reimbursable when billed with a more comprehensive procedure, overlapping services, procedures impossible to perform together during the same operative session, or an E/M service billed the same date as a surgical procedure may be flagged for denial by the code editor.

If an automated edit identifies a potential conflict, the system will consider applicable modifiers before final denial recommendation.
Modifiers specifically taken into consideration include: -25, -57, -59, and site-specific modifiers (e.g., left, right).

The Health Plan retains the right to change, amend or withdraw this policy and to evaluate and adjudicate claims consistent with plan documents, state and federal requirements, and applicable administrative policies. Providers are expected to retain and supply documentation to support modifier usage and the clinical rationale for services billed.

Submission of a claim represents agreement that the provider has complied with this policy and that the documentation is available upon request.
