Provider coding action / maintenance — effective March 1, 2018

Beginning on March 1, 2018, certain ICD-10 diagnosis codes are designated as Social Determinants of Health (SDoH) codes. Providers must code and maintain these SDoH ICD-10 codes on claims and in the medical record where applicable. These codes may be added or updated quarterly; providers should routinely review coding guidance and update billing and clinical documentation practices to ensure accurate capture of SDoH when present.

Effective date: March 1, 2018 — begin using designated SDoH ICD-10 codes on claims and in clinical documentation when applicable.
Maintenance: SDoH ICD-10 codes may be added or updated quarterly; monitor payer communications and code lists for updates.
Documentation: Ensure clinical notes support the selected SDoH ICD-10 codes to avoid denials and to support care coordination.
Billing rule: Include applicable SDoH ICD-10 codes on claims in addition to primary clinical diagnoses as appropriate; follow standard coding conventions for sequencing and modifier use.
